Sepsis carries a 5 times higher mortality than acute myocardial infarction & stroke.
Treat all patients presenting unwell with infection with a high index of suspicion for SEPSIS especially those who are immunocompromised, the very young, old & pregnant.

Dr. Brendan O'Shea, GP and Asst. Prof of Public Health and Primary Care says that there are good things in today's HSE Winter Plan but states that "if this Government was really interested in Sláintecare" it would introduce several thousand nursing home beds rather than 500.
Attending same GP over years is quite good for you !
Although its getting difficult, attending the same GP over years reduces risk of hospitalisation, need for urgent care & mortality.

The HSE and Dept of Health unfortunately have a strong Anti Doctor and Anti Patient bias element within them. They treat Patients, Nurses and Doctors with contempt.

Control through effective population behaviours, level 1-3 lockdown, and high volume test & trace is best strategy.

Great decision - Stephen Donnelly to measure indirect pandemic/ lockdown costs.
All acute hospitals should have website to show real time high vis public display of waiting times for all services as part of this.

Personally I have a lot of difficulty if I’m offered immunisation without my Practice Team....🤔
While I'm truly delighted to see many colleagues receiving their Covid vaccine, it's hard not to be frustrated at the lack of clear plans for GPs + their staff who continue to see patients daily + try to keep patients at home thus protecting hospitals

Heaven forbid the Medical Council should issue a statement that >45 hours per week for NCHDs is unsafe for patient care and for the NCHDs.
And knock me down entirely were the Postgraduate Training Bodies to row in behind them…
All NCHDs who cover extra shifts - either due to illness or unfilled positions in the rota should be paid locum rates. Until this happens, and as long as we keep accepting extra call shifts as standard, the issue of chronic understaffing will never be fixed.
Any health care professional in primary care who already vaccinates in any of the national immunisation programs should automatically qualify on provision of evidence of completion of the horrible HSE Land Module on Covid Immunisations.

We only have 65 GPs per 100,000 popln, also a major deficit relative to NHS, Commonwealth and most other affluent societies, where the number is closer to 95-105.
We are even worse when it comes to Practice Nurses....
Doctors are right about this, and the answer is more GPs and a lot more General Practice Nurses.
Better access to GP led Primary Care is a key social resource for Irish Society, and we are European (even Brexit!) Pariahs in having cost barriers to access to GP care...
At present GPNs in Ireland have no pension, career progression and patchy sick leave.
This makes it almost impossible for younger Nursing Colleagues to consider career in GPN. Until this is fixed - we’re at nothing !

In my practice, we only have a HSE courier at 11 am 4 mornings per week. That means my Chronic Disease blood work has a window of 8 hours in which to get done in any 40 hour week. Could HSE Clinical Design take a look at that eh ?
Dr Mike Ryan observes 'in poorer countries, 90% of front line health care workers are poorly paid Women, largely managed by Men - this has to change.'
Fewer than 20% of these women have internet access.

Dramatic reductions in high-grade cervical dysplasia in the first cohorts of girls vaccinated, now those women have turned 25.
This likely to improve further as time goes by.
Critical Budget Failure highlighted by START Docs - everyone can have a free abortion but 60% who need Contraceptive Care & Primary Care Sexual Healthcare have to pay for it in Ireland (many can't). Fixing this is key to containing numbers of abortions in Ireland...
Hope the IMO takes a serious run at this. NCHDs have been sold repeatedly down the tubes by HSE & Post Graduate Training Bodies colluding to maintain public hospital system in a model of heroic failure, at the expense of our best.

In the interest of public safety would the Medical Council see itself as having a role in relation to Trainees who are consistently exposed to unsafe rosters ? Like removing their hospitals as accredited training sites until they smarten up ? 🤔
If sleep deprivation is combined w a system that discourages doctors to ask for help, it’s easy to see how errors are made

Y'day I watched in horror as this motley crew were herded through Dublin city centre. Their shepherds, a malevolent gang of fascists and conspiracy theorists. They screamed & boo-ed at me as I stood outside Trinity College, the university I graduated from as a medical doctor.
