In most health services planning research we take multi pronged approaches. Some researchers focus on the economics, some on where and what kind of services to deliver, some on evaluating quality of care, and others on patient experiences.
None of this is in the Cass Review!
You could literally run a survey open to ex gids patients and ask if:
A) they are still happy with being trans
B) they are happy with the way they were treated at GIDS
C) if they have any feedback on how their treatment could have been better.

What’s wild to me and yet still unsurprising is that the Cass review did not synthesize evidence on conversion therapy; yet, allegedly Sunak will use the review as the occasion to repeal the conversion therapy band?
This alone should call the process into question.

My research had recently focused on spatial availability & accessibility of gender affirming services at population scale. But even I with zero funding and no political mandate managed to reach 600+ respondents with ease to better understand health services. Cass didn’t even try!

Cass doesn’t follow sound methods for 1) program evaluation (mainstay in public health) 2) evidence synthesis (ratchets well beyond even the flawed but less biased systematic reviews) or 3) health services & policy (see above). So what DID it even research? Seems like it didn’t…

Marginalization isn’t some kind of social capital. A weaponization of identity as a proxy of marginality sure, in some specific contexts.

NEW: By a 6–3 vote, the Supreme Court allows Idaho to enforce its ban on gender-affirming care for minors, except to the specific plaintiffs who brought the case. The three liberals dissent.

Having medical training doesn’t make you an expert in evidence synthesis or systematic reviews. Having clinical mental health training doesn’t make you an expert in the methodology either. Having expertise in systematic reviews doesn’t confer expertise in the subject reviewed.

Universities have to decide. You will continue to see a juxtaposition of sporadic or sustained protests with classes, graduation photos, public events, etc. or you can militarize your campuses, or you can change the status quo. There is no business as usual, that option is GONE.

the rejection i got on a paper i've tried to place in four venues now yet again expresses opinion on method as fact. hard not to be discouraged when there is always a single reviewer (out of usually 3-4) who makes multiple errors without replicating my analysis & it gets rejected

The Canadian examples don’t help your argument. If the entire point is about legality then leave Canada out of it. Limiting applicant pools is legal here. It doesn’t happen nearly as often as you’d think. And you’d be shocked at how many ppl who don’t meet criteria still apply!

Some places have chosen the militarized route, like UCLA/USC/Columbia & others. Some have chosen to change the status quo in rare cases. But some, like UBC, are trying to wait it out.

Thread below gives some (brief!) insight into how systematic reviews work, specifically GRADE tables for assessing quality of evidence
It isn’t that Cass review did it wrong (though they may have!) but that GRADE itself biased in favor of randomized controlled trials
observational studies start as 'low' quality of evidence and RCTs start as high
there are 5 reasons to rate down a study and three to upgrade it
however, those decisions are still left to the judgment of the individual reviewers as this is not a quantitative process

My student evals said there was too much reading.
Most articles were <5k words & a majority were quantitative GIS meaning the hardest part was dissecting the methods.

Totally fair assessment and many of these are really annoying.
I’ll just say that for me as a trans person and someone with chronic illnesses and a lot of healthcare needs these are minuscule trade offs for human rights and actual healthcare!

We need to stop the conflation of the peer reviewed systematic reviews published in a subsidiary journal of BMJ with the overall Cass Review. The latter is, however incomplete, an internal review of NHS services for youth. The former is evidence synthesis (however flawed).
