Paediatric intensive care trainee. REACH Co-Chair, informal junior doctor pay expert,clueless wine snob, fried chicken connoisseur & generally disgruntled human
A large NHS Trust in London has (after months of denying it), finally admitted they have indeed been grossly underpaying their doctors due to HR miscalculations. Expected Response: "Sorry! We will repay you ASAP!" Actual Response....:
@TheBMA
@BMA_JuniorDocs
@NorthThamesJDC
Wow. Let me get this right
@ChelwestFT
A doctor contacts you as you underpaid them when they worked for you. And you blame THEM? Its literally your job, not theirs. Maybe you shouldve picked up errors, or perhaps even not made them?
@BMA_JuniorDocs
@Doctors_Vote
@AliJaneMoore
Today we received our first NHS payslips after rotating. It missing £100s. We work weekends, nights, are eligible for various premia. Why haven't we been paid for this?
@NHSEngland
, why is payroll wrong all the time? Where is the rest of our money?
@BMAResidents
@LondonPaeds
A 3rd
@NHS
trust told in March it'd underpaid
@BMA_JuniorDocs
since 2020. 2w ago HR said itd still take weeks to resolve. Worse-they arent looking to correct for anyone except current staff as its “very difficult”. Knowingly underpay staff for 3 years, too hard to bother repaying
The error arose from an HR dept who simply had no idea how to correctly calculate salaries. I pointed it out months ago & the reply:"I have no need to meet with you as the NHS Employers guidance is quite clear". It is clear. They didnt understand it. Now they owe a LOT of money.
Another London NHS Trust underpaying doctors for 3 years+. Initially acknowledged, only to mysteriously change its mind🤨(NB it actually affected all departments)
The 2nd email was 2 months ago. Still no update. Doctors still being underpaid
@BMA_JuniorDocs
@TheBMA
@Doctors_Vote
The Medical Director of a different trust just DMd me to check it wasn't them. Impressed. Others could learn a lot from that level of personal care and attention.
@realgirl_fieri
Paediatrician here. It's a good question but lots of good reasons why it's a terrible idea. 1) vast majority are viral, even if pain, fever, discharge etc. 2) eben those that are bacterial usually resolve without abxs. 3) Abxs have negatives, often occult. 1/4
If youre worried about being paid correctly, please consider using to check your pay. Accurate salary calculation, payslip visualisation, educational resources, 2024 uplift & lump sum calculator (for individual jobs) (Resident docs in England on 2016 T&Cs)
@PamelaAmeliaK
The new F1s are correct and I'm so proud that they are starting to respect their own time & hours. Agreeing to work for free is deeply harmful, especially when you've then peer pressured everyone else to do so too. Exception Reporting is vital for trusts to staff appropriately.
Another North London Trust found to be knowingly and willingly underpaying its LTFT doctors, but refusing to acknowledge it. How can they keep getting away with this? Please check your work schedules and don't accept anything other than correct pay. These "errors" are everywhere.
@dr_nigel_lane
@NHSEngland
Congrats on completely missing the point. The strikes are because things are getting harder. Yay, they're good for you. Thats not true of your junior colleagues. Failure to support them speaks of a self-centered viewpoint. Not striking is fine, but dont do it for selfish reasons.
@RishiSunak
@Conservatives
You should probably let your pal
@SteveBarclay
know that before he embarrasses himself any further. Pretty sure he said “If people suspend the strikes then we can get round the table and talk” not that long ago. Turns out it was a lie. A dirty tactic. I'm shocked.
@DoctorsVoteUK
Surprised to not see
@DrEilidhMaria
in this list. She has done a huge amount locally and nationally and would be a phenomenal committee chair. Why isn't her work being lauded here?
I have a 5 month old. Is it wrong that I am eagerly awaiting her being old enough to watch
@OfficialBlueyTV
so that I can watch it? Never seen an episode, just heard many good things.
To be clear, this issue also affects several other trusts in London (and likely the UK but outside my "patch"). This trust has at least acknowledged it. Many others are ostriching hard.
If any doctors (those formally known as junior) are interested in learning more about pay and identifying errors themselves, sign up to our mailing list where we will soon be launching some exciting tools. (again apologies for the J word. We'll change it).

What did you do to try and correct them? We don't think we should accept them anymore and so are trying to raise awareness and make improvements. Hopefully in 50 years I won't have to say the same thing. Change only comes if you do something about it.
@ShaunLintern
@WestminsterJane
@BMA_JuniorDocs
What stops the government from adding a pay rise to their manifesto? Announcing policy is relatively important during this period, and a public commitment ends the strikes immediately.

So... Anyone harm caused by the
@theRCN
nursing strikes could have been avoided if
@RishiSunak
and
@SteveBarclay
had been a little more proactive, instead of trying to deny what what were always going to offer. Huh. Shame they spent their time arguing instead of governing.
After some pushing, Im v excited to have meeting next week to help improve the RCPCH Grid application system. Any feedback that I can bring would be hugely appreciated (positive/negative!) Excited to make the process as fair, friendly & transparent as possible. DM or reply here!

He’s done it again.
Dom + the team have created a FREE uplift pay calculator to see how it would work against your WS.
This mans a legend. And I don’t get paid a penny for recommending any of this to you.
85% of resident (formerly junior) doctors underpaid £100s-£1000s per year.
Great to present on widespread underpayment of almost all

to correct it.

There is no peer reviewed evidence that my toddler is unsafe to patients. I'm fact, the incidents per toddler is zero! Far better than any qualified doctor. Time for her to to perform a robotic hemicolectomy!
@DrEilidhMaria
I was an F1 a good few years ago, and I still remember this as one of the most irritating bleeps overnight. That and "you have come come take blood cultures right now because they have a fever and I want to give paracetamol". 🤦
@clifford0584
@Dr_BellaR
It's tricky though, because the dates when doctors rotate are shrouded in mystery and impossible to predict unless you know the EXACT time of Mars-Jupiter alignment, meaning sensible rostering is just an impossible task.
Tomorrow I'm joining a focus group with the RCPCH grid team to help update the grid application web pages.
Does anyone have any idea, concerns or expectations they think should be raised?

Agreed tax codes are an issue but HMRC rectified them eventually. This is very different, and a result of simply paying someone the wrong salary. Nothing to do with taxation.
@jamiereeves1974
Never underestimate the synergistic power of arrogance and incompetence. The software can't fix that. (I'm being flippant but in all seriousness the software can only work with that's inputted. And what was inputted was wrong).
@DrNickTwit
@RCPCHtweets
Please can there be something to evidence someone completing assessments for others? Supervision, teaching and feeding back is important so why not allow us to evidence that? (Also adds some accountability for those who regularly fail to do so...)
@Diktat17
@DPJHodges
You entirely miss the point. It's not that he is wealthy that's the problem. It's that he's equating challenges of people facing significant hardship with not having sky TV. Far more respect if her said "I was lucky and want to give back/ensure others have similar protections"
@EdHutchison
@rosieICM
Yes, a bite! But what is a percentage? It's a symbol indicating "out of 100”. So 21% is quite literally shorthand for 21 out of 100, or 21/100. Which is very much a fraction. God I love pedantry!
@KarenLovesBrie
@NHSEngland
@BMAResidents
@LondonPaeds
Complex! But not the same for doctors. We are paid on an annualised salary, which is very complex but calculated in advance then paid the same each month. In theory. Except it's always wrong
@emmaaaaa8
Couldn't agree more. There is also an incredibly important distinction between being nice and being kind. Far too often we are nice, but not kind.
This is a fantastic thread. If anyone is in any doubt about why these strikes are not only vital for
@BMA_JuniorDocs
but also the
@NHSuk
, and why the approach taken by
@SteveBarclay
@RishiSunak
is a bizarre and unhinged approach, this is a useful read.
The longest NHS strike in history continues
A strike that didn’t need to go ahead
But unfortunately our government is belligerent and ideological
Doctors have had pay cut by more than 31% in real terms since 2008
All we want, is go back to neutral, net zero to ‘08
Just had an excellent meeting with the HR team for a huge lead employer up North, off the back of this poster. They want to proactively identify and correct all pay issues for jnr docs. Other trusts could learn from their willingness to get it right!
We'd like to shine a light 🔦 on employer good practice to encourages others to follow 📈
LTFT trainees, tell us about your good experiences with rotas 📑, pay 💰, leave 🌴(mat, pat, parental, carers etc.), access to learning opportunities 🏫 etc.
@Anniedroid_
Honestly you're sounding incredibly like the recently incumbent and toxic blue political party. "We have nothing to show for ourselves except bitchy infighting and failure, but can you IMAGINE if we weren't here?!”. Pathetic fear mongering when you have nothing left to show.
@DrEilidhMaria
@Joellambert
Exactly what
@DrEilidhMaria
said. However - if you want know whether your salary is (which is often filled with errors and underpayments) - watch this space: I'm hoping to launch something very helpful in the next few weeks.

@LondonPaeds
@RossNieuwoudt
The problem with the DV calculator is it isnt accurate. (It is also convoluted, confusing and time-consuming but less important). It has lots of bits which don’t work, or make incorrect assumptions. It’s also not clear what some bits mean, leading to high risk of user error
@zachary_tait
Don't give in to this nonsense. "I'm really sorry, I work a full time job until I start at the new trust. Here are all my scanned copies, I will bring the originals on the first day". They'll whinge but nothing they can do. Total lack of understanding (not their fault!).
What’s REACH up to next?
Did you know that an estimated 50% of babies with kernicterus in the UK are from Black, South Asian and Other ethnic groups, despite representing only 25% of the population (Baskaran et al 2023)? (1/6)
@RossNieuwoudt
Free is all well and good, but promoting something which is filled with errors is really concerning. I reached out to the team when they published it to highlight this but was (unsurprisingly) ignored.
@Xeon4f145d96s1
@drmattuk
@DoctorsVoteUK
@DrEilidhMaria
You're right, of course, and highlighting the point beautifully. Despite what they claim, DV isn't based on merit, or "who would be best for doctors".
It seems like their primary aim is staying in power at all costs. And that's both a real shame and becoming problematic.

Avoiding getting massively screwed over by your employer by naming them is the point. The people that work there and the

But sweeping implied accusations that everyone involved in improving lives of doctors prior to DV had achieved nothing is completely fine? I would take a look at the way DV has behaved themselves publicly and privately. Glass houses. Adding a smiley face doesn't make it ok 🙃
@realgirl_fieri
Some Individual negatives: they cost money, they lead to personal resistant strains (poorly utilises moteso), can can lead to gastrointestinal issues - altered gut flora, diarrhoea etc, and they're often a pain to get into kids (minor negative tbf) 2/4
@realgirl_fieri
Population negatives: widespread resistance . People using them / giving them to others for inappropriate uses (for colds etc).
People would use them appropriately. Some might not. 3/4

Can we reframe this as "we need to stop ranking and appraising trainees on number of publications as it leads to poor quality research, which harms the evidence base"? Trainees are just doing what they need to. The blame does not lie with them.
