On solid doses of GDMT, needs stability for 4 weeks, could consider ivabradine. SHIFT subgroup showed greatest benefit in those with HR >= 77 bpm so benefit may be less clear here. Costs, one more med with only IIa rec, and SR have been barriers in my clinic
