Yes, failure to recuse 28 USC 455 should AUTOMATICALLY impeach the judge. The judge KNOWS the high stakes of breaking this rule, and they can not deny that they didn't know better. You should not have a choice whether to recuse if the facts are brought forth supporting a recuse.
