@Channel4News Safety science should have been imbedded from the start for a safety branch? Not a good look for an organisation that preaches “no blame” to try and blame previous staff and “historic failings”. The question remains, how does HSIB improve?

@Nikki_HF @pbnes @psmn999 As low as reasonably practicable… not possible. You have to work out what level of risk is acceptable and practical to achieve, what is reasonable… not what is possible cos that might be too cost prohibitive, for example. Some kind of safety management system would be a start

Colour should be redundant - it’s great for highlighting things, but it shouldn’t be the only thing that is different… colour and shape often used together in cockpit displays for example. On that, there’s LOADS of defence standards that would be a great start here

Took my husband for an MRI scan this morning. Had to have forms filled in for him as he’s partially sighted. Now some parts of the NHS know he’s severely visually impaired… but this knowledge is not shared. And don’t get me started on the car park ticket machines… #humanfactors

I once spoke to someone at the DHSC about the packaging risk and they said, “Can’t we just get people to check better..?” I sighed and explained that they had actually tried that, so maybe it wasn’t the solution…

It’s international women’s day! I recommend following @PayGapApp which picks up tweets from companies talking about how important women are, then posting the pay gap in that organisation. Fascinating.
In this organisation, women's median hourly pay is 36.5% lower than men's. The pay gap is 2.7 percentage points wider than the previous year.
