Gem 17% 5y OS in ESPAC3, 28 % with Gem Cape in ESPAC4, the 6% 5 year OS in the immediate surgery arm looks bleak even after taking into account only 51% received the post op chemo. Does the NA Rx 20% 5y OS look good only because upfront surgery did unexpectedly badly?

@MarcBesselink @IHPBA @HoggNDMD @EAHPBA @YouppiePancreas Now enthusiastic surgeons need to find a way to convince the government and insurance to pay for the Porche to replace the Ford model T especially in LMIC countries!

@Shrikhande_SV @IHPBA @TataMemorial @SirohiBhawna @drjagannath @drsadiqs Congratulations to Shailesh, Bhavna and Anil for this balanced overview. Early detection and differentiation of inflammatory thickening and cancer preoperatively is fertile ground for research.

Good message on need for Gerota fascia resection. Interestingly could not find pN0 vs pN1 numbers though LNR is focussed upon. Median node yield is 1 so 50%+ LN positive. Sure N0 was looked at in a survival prediction analysis. Multi visceral details probably a separate paper..
Predictors of survival after distal pancreatectomy for #PancreaticCancer retrosp cohort of 1200pts, 11 countries: Gerota’s fascia resection, R0 resection, decreased lymph node ratio, adj chemo. Ongoing DIPLOMA trial includes routine Gerota resection.

@sanjaynagral @cspramesh We await the tocilizumab results with great interest too. Possible because of the adaptive design of RECOVERY. Results will always be credible because of transparent methodology and data presentation. Facts will always be the facts. Interpretation can be moulded though...

@cspramesh Advantage of huge pragmatic trials..very large numbers makes this believable. Credible hypothesis too as to why this is happening. Our physicians treating COVID already using steroids this way anyways..but great to have solid evidence they were correct.

@sanjaynagral Sad it cannot be done online....if one can book and reserve an airplane seat with such great efficiency weeks in advance surely a hospital bed app cannot be too difficult

@WormlockHolmes @cspramesh @spkalantri @CancerGridIndia We can only do good trials during a crisis when we have done so in times of peace. RECOVERY works because of the system creating it and the culture and work ethic inculcated over decades
