In this teaching video, we propose that all Whipple resections should be by an SMA first approach.
Just the way all rectal resections should be via a TME and nothing less.
Unless we do it well in resectable disease, we cannot do vascular resections well!

Perfect. In fact, all solid tumours, irrespective of stage, are best seen first by a surgeon & the rest of the multidisciplinary team depending on individual case. We need to be good human beings, good clinicians and finally good surgeons in that order.

Couldn’t agree more with you. My fellows and peers are going to have a quiet smile. “Attention to details” is something they hear day in day out 😊👍also, to quote from the book Godfather “accidents don’t happen to people who consider them as personal insults”...
“Details and Complications” is what define great surgeons. Great surgeons pay attention to details to prevent complications. And when complications occur they don’t ignore them, they pay attention to details to make the complications better.

Never underestimate “relative simple part of the whipple” - the cholecystectomy. Be at your best and always do a double gauze piece test. One for GB fossa and one for the HJ. Such an approach has served our team well
Don’t forget to check GB fossa at the end of whipple resection, else it can bring you to grief at times
