In this teaching video, we propose that all Whipple resections should be by an SMA first approach.
Just the way all rectal resections should be via a TME and nothing less.
Unless we do it well in resectable disease, we cannot do vascular resections well!
Here I start my twitter innings... Centralisation is the key to improve standards in complex cancer surgery. Difficult task but certainly desirable for a country like India.

Robotic continuous suture PJ with V Lock sutures in uncommon for us. Pancreas texture and “the way it lends to the jejunum” can sometimes make this feasible.
Teaching and transferring skills is important. Cultivating the right attitude and sustaining growth and joy as a good surgeon and human being is perhaps even more important over time 😊👍🙏🏻

Couldn’t agree more with you. My fellows and peers are going to have a quiet smile. “Attention to details” is something they hear day in day out 😊👍also, to quote from the book Godfather “accidents don’t happen to people who consider them as personal insults”...
“Details and Complications” is what define great surgeons. Great surgeons pay attention to details to prevent complications. And when complications occur they don’t ignore them, they pay attention to details to make the complications better.
Never underestimate “relative simple part of the whipple” - the cholecystectomy. Be at your best and always do a double gauze piece test. One for GB fossa and one for the HJ. Such an approach has served our team well

5.8% isolated Local recurrence/ 12.6%LR + distant mets / 25% isolated DM. Big difference in survival between LR VS DM. SMA clearance is important even in periampullary cancer! Congratulations and great work
