Hi
#MedTwitter
! My name is Nawaz Safdar and I’m a final year med student at the Uni of Leeds
@LeedsMedHealth
. I’m applying
#InternalMedicine
during
#Match2024
and would love to connect with you! I love endurance running but as of late my mouth’s been more active than my legs 🌯
#MedTwitter
there's been some interest regarding the process for
#Match2024
and
#USMLE
from the perspective of a UK student/doctor so I'll try and briefly summarise what I did. This was my experience and almost everyone seems to have a different one. 1/n
My face hurt from all the smiling but yesterday I graduated from
@LeedsMedHealth
as Dr. Nawaz Safdar MBChB (Hons). A big thank you to my family, community, and friends for helping me get here… excited to begin working as a doctor
@LeedsHospitals
@RobJimFleming
I spent a lot of time on the fence trying to decide what would be a good choice for me… with the way things stand at the moment, the choice is very clear
Had a great time presenting our work on the (microphone) vascular markers in response to interrupting prolonged sitting in T1D and also some observational activity data at
#DUKPC24
before I jet off to begin my training at
@ImPennsy
later this year

We surveyed 240 final years across 26 UK medical schools and found a need for more vascular surgery exposure, higher confidence in vascular examinations, and increased knowledge of vascular conditions. Find out more in our latest publication:
🚨Do older people (>80 years) benefit from cardiac resynchronisation therapy as much as those who are younger?

Excited to share the publication of our review on coagulation in diabetes, and current and future targets for thrombotic risk reduction in these patients. Thanks

7/n. In conclusion, the USMLE tests your medical knowledge but mostly tests your test taking ability. You get what you put into it and since Step 1 is pass/fail, its now very accessible, even to those who have been away from med school for a while now. Drop me a dm for more info
2/n. Full disclaimer, this process is expensive and we are considered IMGs in the US so there is definitely some bias against us when applying. So before you start, do your own research into the process...
Patients with unplanned vascular admissions are receiving insufficient end of life care based on UK guidance despite having high mortality

4/n. 6 months later I sat the 8-hour exam knowing that it would hold me in good stead going into 3rd year. After the exam, that year was fairly straightforward for me academically since the Step 1 is more "difficult" than most exams that you'll do in med school (i went to leeds)
3/n. When lockdown began I was a second year student at the end of pre-clins and I was presented with a golden op to study uninterrupted for Step 1 which was the one that mattered (past tense) and tests you heavily on pre-clinical science (yes Krebs cycle I'm looking at you)...
Hi I’m Nawaz, a soon-to-be 4th year medical student with an interest in IR and research. I'm really keen to improve awareness of IR in medical schools and to be involved with BSIRT collaborative research initiatives

Very excited to have been awarded the Richard King Trainee Award for Best Trainee Publication in Genetics in Medicine at the 2024 American College of Medical Genetics. I am so lucky to be part of this field as we move forward to affect the change our patients deserve.

5/n. I took some time off to work on other aspects of the application (research, presentations, etc) during 4th year and by now my clinical understanding of medicine was improving. Started study for Step 2 during late 4th year and early 5th year...
✅Our findings support the provision of CRT in selected older patients with a reasonable life expectancy and medically optimised heart failure. Referrals shouldn't be deferred solely based on age
Solid team effort

6/n. By now Step 1, the biggest barrier to people doing the USMLE, was pass/fail and the onus has shifted to Step 2 which is a clinical exam and is a more difficult version of UK med school finals. Again after having done it my final year of med school has been relaxed...
Of the tuberculosis drugs, pyrazinamide, isoniazid and ethambutol but NOT rifampicin have been associated with arthritic side effects in isolation❗️🫳🏽❗️
Here, we describe a case of rifampicin-induced arthritis which was not a part of an SLE-like syndrome
🚨Time to heart failure hospitalisation was similar (p=0.28) and 10-year all-cause survival was less favourable for older patients (p<0.001). Complications were infrequent and similar across groups
5/6
🚨Age was not associated with response to CRT either as a categorical variable (67.2% v 71.2% v 62.6% responders in the <70, 70–79 and ≥ 80 cohorts, respectively, P = 0.43), or when described as a continuous variable (OR 1.00 per year [95% CI: 0.98, 1.02]; P =0. 80)
4/6
Older patients were more likely to have❌worse symptomatic status,⬆️more co-morbidities, and⬇️less optimal guideline-directed medical therapy. ECG findings and left ventricular function were🟰similar across the groups
3/6
Randomized crossover trial with 32 adults with T1D who completed two 7-h laboratory visits separated by >7 days. Participants either:
a. Seated for 7 h (SIT) 🧘
b. Interrupted sitting with 3-min bouts of self-paced walking at 30-min intervals (SIT-LESS) 🚶

Match is for residency and fellowship. Generally no, but a bill passed in the state of Tennessee now allows IMGs to work as attendings without having to retrain. Not sure about the board certification!
@TheSiddRaj
Maybe the knowledge gaps yes but still lots of work to be done to bridge the gap re exposure, physical examination, and other barriers to careers in surgery
The next🚶*step*🚶is to investigate whether this intervention is effective at⏲️delaying ⏳the progression of established complications and whether this can be applied remotely within the community🏡

I thought that the content we were learning at med school was nowhere near enough to what you need for step. However, as I said in my other post, the reverse is true where the UK curricula seem fairly straightforward after doing the UsMLE. Plus I think it taught me how to study
