@ashrafelfiky1 @shafinyc On the differential for sure but projections were not circumferential (more like Mohawk)! The infiltrate was nodular on bone marrow with some intrasinusoidal activity. Neg for cyclind1, cd25, cd123, cd103, cd11c and had del 7q so great for splenic marginal zone lymphoma.

Very cool work from @JeffTownsend and his lab that was wonderful to be a part of #hemepath.
Sequence analyses of pediatric acute lymphoblastic leukemia (ALL) have identified somatic genetic mutations as therapeutic targets. Typically, they've been prioritized by mutation prevalence. Much better: prioritize by cancer effect!.

Agree with Danielle, total tumor cells. However, because it has been thought that there's some off-target activity, questions have been raised as to whether we should report %of tumor cells as well as %of non-tumor cells (a bit like some other companion diagnostics) #HemepathJC.

Pathologists: how do you haul your slides from office & back? We use beer coolers and mine was on its last Velcro strap so I used a rubber band to remedy for a month. But it completely collapsed last wk (too many cases)! And yes, my shoes match the bag at heels. #hemepath

@vaskannan Great question! We thought it was just a nice Castleman at first, but flow found a small population of double pos (CD4/CD8) T cells and there is that expanded rim of lymphocytes in the periphery. This led us down the TdT, CD1a staining pathway.

@DitaGratzinger Thank you Dita; it would mean a lot to me to hear your thoughts on the paper. Yes I believe it does pick up on promonos. The few discrepancies were primarily in over-count rather than under. Would be interested in multi-inst validation for sure!.

@meryemkubrat We weren’t sure either. But “EBV+ large B cell lymphoma” seemed true to the tissue with a comment that it appeared morphologically (and clinically) like PMBL. After discussion they treated w PMBL regimen and pt has done very well.

@Dr_AmerZeidan Ichigo Daifuku (strawberry mochi). The layer in between is red bean paste. Outer layer is soft room temp mochi. The strawberry that they use for this super sweet. The best I've had was in Tokyo but Hachiroku in New Haven does a great version too!

@AadilAhmedMD @MaracajaDLV @ChrisTormey3 Upon last review for a different study, CD30 semi-quantification by IHC is standard. Flow showed lower sensitivity (some attribute to the larger tumor cells not getting analyzed). CD30 mRNA quant was more sensitive but not really clinical test.

@lbsmithmd THIS. I’ve had complicated marrow consults for MDS w about a brick of paperwork detailing the molecular genetics performed and the many nonspecific findings. And after hunting high and low, have uncovered normal cbc! Biopsy initially done for lymphoma staging.
