CT surgeon, Momma to 2 hooligans, Scientist, Educator, Currently appreciating the wonder of fantastic mentorship & how much fun cardiac surgery really is :)
There were several medical students interested in this AM’s chat-thought I’d offer up another something useful for that 20min.
🧵 for knot tying - the basics
There are one ☝️ handed knots which most of us prefer, BUT the two handed 🙌 surgeons knot is the gate keeping skill
Dear
#medtwitter
,
Please give me your best (non-serious) explanation for the cow that greets me on the Epic Login page.
Sincerely,
Confused academic surgeon
in OR w/
@JCoselli_MD
scene: master surgeon, particularly terrible tissue & single suture pulling through
he says: “wonder if I pulled up too much or didn’t take enough tissue on the bite”
he didn’t say: terrible tissue, bad instrument, poor exposure
Ultimate responsibility.
Basics of knot tying part 2:
So the 2 handed 🙌 knot - you’ve done the first part and now what you’ve got 👀 something like this ⬇️ (notice my finger tip is a millimeter from the knot pushing down beside the 🪢 not on top of it)
Tomorrow @ 11am we are going to chat with some of the big names in CT surgery about the Ross procedure. We noticed our group is pretty heterogeneous and we ❤️ that so we thought we’d offer a little primer so everyone can engage even if you don’t live in the 🫀😷 world.
What if I suggested that we should train CT surgeons like professional athletes b/c our performance in response to stress is quite similar? Then what if I told you that we may have a way to do it?

Who: Pts w/ Thoracoabdominal surgeries (the one where u basically ✂️the human in 1/2-👀 @ that incision! Big incision=lots to close for an intern😉)

Update:
@AATSED
leadership conference➡️case scenarios➡️female surgeon reported for unprofessional comments
@DukeCTSurgery
chief stood & said it’s his job to make sure she isn’t being reported for something Sr. male partners do regularly & are not reported.

Look look look!!!! so many young female students & trainees interested in CT Surgery!!! They flew across the country to spend a day with us🫠
So incredibly cool!!!

Basic knot tying Part 3: getting a little fancy (one handed knots) 🎩💃
So tying 🙌 knots is a pretty classic way to judge where a student is & where to start teaching from. Now that you are able to tie a 🙌 🪢 like a pro let’s step it up.

@Askflorence
Reminds me of my 20 yo son’s favorite joke when he was 6- what do you call a cow laying down? Ground beef 😂 (it was also that bad when he was 6)
Just listened to Dr. Tirone David nonchalantly present that young patients shouldn’t have bovine pericardium for commando recon (b/c calcified patches =fragile…yup) We should be reconstructing with double graft/valve. 🤯 how cool is that?!?

@AATSHQ
This!
He’s literally always on call b/c he is everyone’s bail out call
His cell is a telemarketing nightmare. He always answers on off chance a surgeon has a pt in trouble… worldwide
If hard thing=right thing for pt, buckle up. We’re doing hard things today.
I have never seen anyone more dedicated to the well-being of his patients. Patients always came first. He worked super-human hours to save the lives of countless patients-many were turned down by other surgeons because of the risk/complexity of the surgery. He was their last hope
What do you do when a mentor who has always been a boss in the OR gets promoted to THE boss? Order the appropriate signage, of course! Congratulations

Not a single trial has ever reported a complete revascularization rate that is that low with CABG. A rate of 35.7% is implausible and incompatible with real world practice. I call for an independent inquiry and review of the data and methodology.
Really cool 🧵- @ med students

, just wrap your head around the idea that heart transplants lose their vagal connection 👀 the autonomic stuffs is cool way to remember the basics of ANS but when I learned that transplanted hearts are basically neuro-stupid = 🤯
Can heart transplant recipients experience vasovagal syncope??
This came up during my time in the CCU, here's a short 🧵 investigating this question. Special thanks to
@AHajduczok
of
@TJHeartFellows
.
Let's first start with how vasovagal syncope normally occurs...
Huge ❤️ to my co-fellows!These 2 reminded me daily that my humanity had as much value as my intelligence. They made it possible to be trained (relentlessly)by a world class program & still maintain the ability 2 laugh at myself. Thank-u isn’t enough.

My favorite thing about

This isn’t easy or everyone would do it.
how u handle failure determines who you’ve chose to become. When u do please forgive yourself & talk about it w/ your juniors. That’s how we build a resilient field

Here’s the step-by-step
1- go 👀 pt ➡️make sure it’s not something else
2- call aortic fellow (this =why they are doing dedicated yr 😎)
3- algorithm
4- if u don’t know, call a boss! We’d rather be awake at 2am than 😳 at 7am
