In summary, although insulin resistance is the major cause of AN, it can be seen in a wide range of conditions. In the absence of classic metabolic risk factors, assess the lesions severity and the site of involvement (R/O malignancy) and also ask for family and drug Hx. (12/12)
Clues that would favor acanthosis nigricans associated with malignancy include:
1⃣ Severe lesions with rapid spread
2⃣More prominent mucosal lesions
3⃣Presence of other cutenaous manifestations of cancer (e.g., triple palms)

Hypervirulent K. pneumonia strains produce a "hypercapsule" consisting of exopolysaccharides which gives the colonies their classic mucoid appearance. (3/n)
A small amount of gentamicin can also directly enter the cytosol and lead to cell necrosis through massive proteolysis or act on mitochondria triggering the translocation of cytochrome c leading to cell apoptosis. (5/n)
In summary, the mucoid capsule of K. pneumoniae helps the bacteria evade the immune response through ⬇️phagocytosis, ⬇️availability of antimicrobial peptides, ⬇️inflammatory response, and stoping the complement-mediated lysis. (10/10)
The answer is straight-forward: DO NOT STOP!
HIV is notorious for establishing a latent infection in a wide range of cell populations inside body. The lymphoid tissue is the major reservoir. (2/n)
Therefore, as long as we don't have any means to eradicate these latent reservoirs, we don't have a cure and those w/ HIV should continue taking ART. (6/6)
Finally, acanthosis nigricans can also be seen as a part of various genetic syndromes. These syndromes are generally characterized by the production of antibodies against insulin receptor or defects in the insulin/fibroblast growth factor receptors.

How do these lesions form?
It's most commonly seen in association with insulin resistance. So elevated insulin levels could play a role. However, there are other situations where acanthosis nigricans is seen in the absence of hyperinsulinemia (e.g. cancer).

In malignancy-associated acanthosis nigiricans, ⬆️ levels of transforming growth factor (TGF-α) produced by cancer cells ➡️ interact with EGFR and FGFR ➡️ proliferation of basal epidermal cells ➡️ keratinocytes proliferation.
