I tell patients if you are here on Friday, you will be here on Monday. Place shuts down for everything except physician work. No social workers, no case managers, no bed supervisors. Surprised they feed my patients sometimes.

We are NOT required to accept everyone as our patients, provided we don’t discriminate based on sex, religion, etc. We must provide emergency treatments, but can discharge anyone with 30 days notice for any behavior that we think is inappropriate. Our safety is important also!

Truth is that most physicians can’t read a balance sheet, don’t know how to calculate EBITDA, and don’t know the difference between an PA and P&L. PE comes using terms like NPV, current ratios ,and docs glass over. We should have financial literacy rotation as part of training.

That is Charcot osteoarthropathy. Radiologists will note the collapse and bone change, call it osteomyelitis and MRI will have marrow edema so they also call it osteo. Bone biopsy and culture is the ONLY way to make sure. These do not always need amputations! Save a limb

Have used Metanx for years. Got or miss. Can be expensive and difficult to get as it is a “medical food” not a medication. BTW, all neuropathic patients should be seen by podiatry for proper shoe/insert or deformity management before they get a DFU or similar.

Everyone has a different patient population. I personally discharge someone every 2 or 3 yes because most people aren’t the problem. It’s rare. We do d/c at least 1-2 patients a year from my entire practice (we are 12 docs). We don’t deserve to get abused for trying to help.

It is a matter of control. Each state has its own medical bureaucracies with boards for Medical, Dental, PT, etc. if we had a national standard, what would all of the people working for these boards do? Investigators, managers, lawyers, etc.

While it's true that many pieces of information can be looked up quickly, having medical students memorize antibodies builds a foundation of knowledge that they need. You have to know what to order when you need to test for something.
