I left Gaza the day before yesterday, after 8 weeks of working beside my Palestinian colleagues. In my first week I witnessed the murder by Israeli forces of two fishermen on the beach, one shot through the neck and one shot through the heart still holding his cast net.

5 days after the start of Ukraine war, UK opened 2 new, safe entry paths for refugees. 1 was for Ukrainians who have British citizen relatives. 12 days after start of #Sudan war, UK has sent border control to the airfield to turn away NHS doctors, elderly mothers of citizens

This has been my 7th war with MSF and it has been unique - Iraq, Syria, South Sudan, Ukraine, Tigray, Sudan - the population could move away for safety. Not so in Gaza. The opposite in fact. Gazans are herded into a space not fit for survival.

All in all, lots of ups & downs. I’m sad the NHS is being ground into failure. I couldn’t hack it there. I have huge admiration for my colleagues & friends still working there under huge strain. But I’m very happy doing all my other doctor things. Here’s to another 20yrs 🥂

Nothing good happens in comms blackouts. Tigray last year, Darfur since May. Now Gaza. If it wasn’t for Gazans getting their stories out we’d only have CNN/ABC correspondents lying in ditches somewhere outside Tel Aviv to tell us their news

Finally, 2023. What am I doing now? Well, I’m a trustee of the RCEM since two years ago. I did 3 months for cholera in Malawi with the WHO, and right now I’m doing 6 weeks of that working from home. I also squeezed in 1 month with MSF in Sudan in May.

I went from Haiti to cruise ship doctor via Syrian civil war in the space of 18months. All painful knees and indigestion. Awful job. Indignant that so many died horrible deaths and here these people had open access to a doctor for such trivialities, I burnt out with anger.

2009 first time with MSF: 6 months, Basra, Iraq. Teaching BLS, ALS, ATLS and bringing in triage and mass casualty processes to the DGH ED. Patient care was single sex. Nurses were under-utilised. The space wasn’t fit for purpose. HUGE job. Epic. And right up my street.

It was a bit of a whirlwind after that. By now I was making TV series for Al Jazeera and BBC. Japan, Brazil, USA, Jordan and many more. 2 BAFTA & 1 Emmy nominations. 2 FPA nominations & 1 FPA win for a film I made in Gaza. It seemed I had a second career as a TV presenter!

Sudan has a high Caesarian section rate. Because of the authorities’ ban on surgical supply reaching Khartoum we have already had to close one surgical hospital (Basha’ir) last month. Our last remaining surgical hospital in Khartoum (Turkish) now has less than one month

2022: 2 months Chad, 4 months Ukraine and 5 months Ethiopia. Mobile clinics in the sun by a river or underground in the Kharkiv metro. Using donkeys for transport or cycling to meetings. I was very happy and knew I could never go back to the NHS again, which was daunting.

They did split up. Cousin is still at Argeen. All others went on to Aswan. Beginning to look like he may have to go back to Halfa…very difficult decision to make. Might sit it out another day at Argeen before giving up.

Friend sent me this from London yesterday. My dad is Sudanese 🇸🇩 My mum in Iranian 🇮🇷 They met and married in Cambridge. My sister and I are British. London is my home. I look foreign in all my three countries, a child of the world. Today that feels wonderful. 🙌🏽

While I was away, I found out that I won’t be given security clearances for the new job. GUTTED. Back to ChelWest ED it was then. Hang on. I want to do a masters. Got a place at LSHTM/LSE for the HPPF, defer a year and got a job as a cruise ship doctor to save lots of money.

I didn’t last long. By July I was signed off sick. I resigned in September and applied to join the MSF emergency desk. I figured the recurring theme with me has been that I’m happy at MSF, even if it’s tough. I definitely can’t handle the NHS. Cue all of 2022 abroad…

I did my house jobs and started surgical training, intending to be a T&O surgeon, with the hope of one day join MSF. Being a consultant in the NHS was simply never an objective of mine even though we all have it drummed into us that that’s all there is.

Later that year, I answer MSF’s call to go to West Africa. It turns out to be a huge epidemic. Ufff this was tough. But, after Haiti, I recognised it now. I let myself cry when I felt like it. A little boy’s death broke my heart. I made a TV doc about it, which went global.

Swiftly followed by MSF: 6 months, Port-au-Prince, Haiti Clinical lead for ED, internal medicine and ITU in 100-bed hospital in Cité Soleil. Uffff this was tough. We worked hard, partied harder. 7 days a week. Now I see that all that alcohol was a negative coping mechanism

I realised T&O wasn’t for me when I was more interested in sorting out the post-op AF than being in theatre! So I got a middle grade job in ‘A&E’ as it was called then and did the DTMH at LSHTM, a requirement for joining MSF. When I applied to MSF, I was rejected.

Then cholera came…I’d never seen so much avoidable death. Pic from day 1 and last day. Felt like I’d been run over by MSF. So before I’d even left Haiti, I got a job in Australia doing HEMS. When I got home, I got cold feet and cancelled my flight to Oz. Went back to NHS.

By 2014 I’m feeling up to MSF again: 1month, South Sudan. Helicopter mobile clinics in the civil war. I get a second wave of enjoying doctoring. Ebola is declared in West Africa. So few people have Ebola experience that the nurse I’m with is pulled out and sent there. Unusual.

Got my London job in Worthing. REALLY enjoyed ACCS there. Did MRCP, because in those days we were allowed to only do FCEM at the end. By the end of ACCS I wanted a year out for MSF. Deanery said “run through, or resign your number”. Resignation letter went in the same day.

It’s now 2011, the Arab spring. I’m hating EM. So I apply successfully to join the UK Foreign Office in the fast stream diplomatic service. I take “one last MSF stint” in Homs, Syria. Was too dangerous to go in, so worked from Lebanon. Started up trauma supply networks.

I put my stethoscope down, resigned, went home. I cancelled my masters. It was 6 months before I could even conceive of working again. When I did, I started 1 day/wk in ED again. It’s 2013. I feel good doing trauma in a busy MTC. After 3 months, I find balance at 3 days/wk

Happened to my family and my friend’s family. NatWest and Barclays. Were told do with dual nationality (Sudan in our case, Nigeria in his). Seemed defensible because of possible dubious financial flows in those countries BUT what’s that got to do with some Brits in UK??!

Though I technically qualified, they said I should get more experience. So I went back to my NHS A&E middle grade job. 2006/7 MMC came along. Those in my year were advised not to apply to ST3 but to ST1 instead, in case we’d be forced to Article 13 at the end. Applied for EM

In 2016 I spent time with people living in the Calais Jungle and also worked on an MSF Mediterranean Search&Rescue ship. I saw the UK invest in higher walls at the motorway around the camp (see pic) & French police use tear gas and force to deter people from attempting entry /1
