Persistent "offloading" strategies are still evident 9-12 months following ACLR. The deficits are apparent in higher level loading tasks (e.g. Tuck jump) as assessed in this study.

The irony of the advice to run slowly to get faster is that many of today's best marathoners were champions on the track with amazing speed. Kipchoge, Bekele, Cheruiyot, Dibaba, Hassan etc. Speed is an essential attribute for endurance.

We are so concerned with the "optimal" type/intensity/duration of exercise that we forget that the biggest public health benefit is from helping those least active to do a bit more.

@tomgoom Not too dissimilar to a belief that something is effective because of biological plausibility.

@neuromanter @pl_valenzuela Doing 7 reps with a 15RM weight isn't really similar to any previous study looking at the performance benefits of heavy resistance training.

@MattSiniscalchi It is usually based on the point where the curve levels off - the relationship between MVPA & all cause mortality.

@MattSiniscalchi We really set a minimum amount of exercise. It's all beneficial.

@MattSiniscalchi Yep but public health recommendation shouldn't be based on surrogate endpoints.

@MattSiniscalchi Yes, the study doesn't tell us that we should aim for 4-5 times per week of exercise. Only if we rely on surrogate endpoints.

There is no "minimum" amount of physical activity that someone should do. Facilitate & encourage people to do a little bit more. The evidence is clear. It is hugely beneficial.

"Often the ‘true’ endpoint is one with clinical importance to the patient, for example, mortality or a major clinical outcome, while a surrogate is one biologically closer to the process of disease, for example, ejection fraction. "

The data provided do not support the idea that 150 mins is "not enough". Public health policy should be based on tangible outcomes like all cause mortality, days lived with disability etc.

4–5 days of aerobic exercise per week may be the “sweet spot” for preserving a youthful heart structure as you age This frequency helps slow the natural shrinking and stiffening of the heart, preserving its elasticity Anything less—including the often-recommended 150 minutes per week—likely falls short of providing optimal protection There are definitely many, many benefits to only exercising 2-3 days a week (metabolic, brain health, etc.), but maintaining youthful cardiovascular compliance and structure into old age seems to require more effort

@DerekGriffin86 When I ran I did a lot of miles even going up to 180 miles for the week I found that it had a negative effect on running faster so dropping it down to 150 for the week the status quo was balanced out
