It’s 30 years since I hung up my stethoscope at #stvincentsuniversityhospital and I don’t regret for a moment going into #ophthalmology and #retina . Most went diverse ways, some lost forever, some stars but we got a great start from @UCDMedicine that allowed us do any branch

The Royal Victoria Eye & Ear Hospital is the largest eye unit in Ireland. Our annual budget is approx €40 million. We ran a small deficit last year of €1.5 million, under 4% of our annual budget despite a 7% increase in activity looking after a greater number of cataracts, etc

Abortion is NOT a treatment for diabetic eye disease in pregnancy #together4yes . Shocking scaremongering. Effective eye treatments by my retinal colleagues and I mean that no pregnant diabetic woman risks blindness. Take eye advice from eye specialists and not Ob/Gyn campaigners

@annedohertypsy Puzzled by this reference to accelerated diabetic retinopathy during pregnancy. Termination of pregnancy is never an intervention for this but strict blood sugar/blood pressure control, and sometimes laser..and that's from more than 20yrs experience as a retinal specialist

If you're undecided about #8thref then don't let health risk scaremongering about blindness and diabetic pregnancy by @Together4yes influence you. My colleagues @eyedoctorsIRL have very effective treatments for diabetic eye disease in pregnancy...and abortion isn't one of them.

@RMcGreevy1301 No, but they are not buying properties there. They are renting and getting paid very well. Rental property is slightly more available and affordable. And it’s the herd and FOMO effect..all my friends are there on ‘gap year(s)’ so I should go too.

@ProfPayEquality That’s a good thread. As specialists we should restrict comments on any medical area that is not within our field of expertise. I’m referring to ED consultants/ immunology scientists etc opining as experts on public health. There is a reason that Public Health is a specialty.

For the first time ever, the HSE did not cover the deficit and made us roll it over to this year. Our population has grown nearly 20% over the past 20 years, partic over 65s and there is a massive demand on eye services. We now have to curtail activity with theatre closures

Prof Coakley was an excellent lecturer accompanied by essential illustrations of anatomical structural relationships. He gave us our introductory initial lecture in Pre-Med, advising any who had come to Medicine to make money to leave now, pointing at the door. High impact.

If there’s a 4 day delay in processing swab tests (from swab to result), surely that means a higher false negative rate? Asking for a frontline surgeon who has had to stop on call and defer non elective surgery. @nvrlucdireland

@Together4yes Is Peter Boylan really suggesting that a termination is a valid intervention for diabetic pregnant women to reduce the risk of medical complications like blindness? That is completely inappropriate and not the accepted standard of care for diabetic retinopathy in pregnancy

@IrwinGill @drmickmolloy This is one of the biggest challenges for us as hospital consultants..there is no flexibility, locum cover etc. Self isolate or fall ill yourself...the system collapses in fragile ‘bare bones’ cover as it is.

@DrHenryMarsh I trained and worked in the NHS for 4 years in the late 90s..and did the same for periods in the US, IRL and Aus. The training and organisation were somewhat better in the UK but the clinical patient priority maintained by consultant autonomy with MDT focus better in IRL

Glad I did my first inspection yesterday..5C now, too cold for foraging flights. Not bad, no colony losses but big variance in performance from 4 frames of brood to virtually none with a poorly performing queen and already a supercedure cell #beekeeping #nativeirishbees #honey

So now there are empty beds in both public and esp private hospitals.

Very much of its time but the most striking for me is how articulate and mannerly these working class builders and labourers were..products of good basic Irish education system but couldn’t get jobs at home. Very different now.

@mccarthymt7 No, the whole concept of physician associates is borderline exploitation and gradually undermines the role of doctors. You cannot have an accelerated 2 year program tacked onto any previous discipline and expect clinical equivalence, responsibility or competency to medical school

@Antcon7062 @SimonHarrisTD @HSE_HR I’m on call 24th-30th inclusive, one week in 3 throughout the year for past 17 years. That means one week on call every year through what others describe as the Christmas break. Guaranteed to be doing emergency retinal surgery. This demeaning and cheap shot is dispiriting.
