This week, we had the chance to welcome Bobbi Symes, a leading champion of social prescribing for healthy aging in BC. With Bobbi, our team had the privilege of visiting @rexdale_chc and its attached Seniors Active Living Centre (SALC) to hear firsthand how their #socialprescribing programs are improving lives. From supporting seniors in staying active and connected to helping families navigate the health and social care system, their work is making a real impact—ensuring people get the support they need, including access to primary health care. We also had the opportunity to visit @UnityHealthTO's St. Michael's Hospital, where we connected with their team to learn more about how they’re integrating social prescribing into clinical settings. It’s inspiring to see how health and community care providers are working together to bridge gaps and ensure people receive holistic, person-centered support. A huge thank you to the Rexdale CHC team, as well as the dedicated Academic Family Health Team at St. Michael’s Hospital, for hosting us and sharing their work. Seeing social prescribing in action reinforces why this movement matters—it’s about real people, real connections, and better health for all.
