I’ll tell you a story. When I was in residency a 200+ pound man was brought into the emergency department for agitation. He was uncuffed by NYPD and released into a room with sliding glass doors. He was sweating, shouting, punching walls, and punching himself. 1/

Realized I use "I have kids/husband" as reasoning for not being able to make evening and weekend meetings. This explanation could imply that people who are single or without dependents should be willing to spend off-hours doing work. That's not fair. We ALL need space & time off.

I hear from POC residency applicants about layers of isolation from virtual interviews. It reminds me of my own experience applying to programs, sitting around packed tables as the only applicant of color. We hope "Interviewed while Black" characterizes that common experience.

I am not threatened in the same way or frequency as law enforcement. However, I am threatened enough to understand fear. Harming the patient is not our default. I hope we, healthcare providers and law enforcement, can exchange stories, strategies, & skills to work together. 6/

There are strategies to control our response to threats. Often it is giving time & space. Often it is physical restraints w/ safe positioning. Often its medication to cause sedation. Never, not even once should it be murder. We are not perfect & we are not above the law. 5/

Awful seeing unvaccinated patients months after COVID has resolved w/ lingering shortness of breath. Negative EKGs, Labs, CXRs, CTPEs, ultrasounds.. Them: Doc if I get vaccinated now will it take my symptoms away? I can't live like this forever So hard to see their desperation.

Excited bc this weekend I get to celebrate my 6 year wedding anniversary with my favorite husband. Started dating 13+ years ago & now 3 kids later...we are still in 💚🧡💛 ...though very tired & overwhelmed by daycare costs😂 Grateful for it all, the good and hard.

After 10 years of being interviewed or conducting interviews in medicine I believe URM applicants should be interviewed by at least 1 URM interviewer. Ideally, this is a faculty member who is appropriately compensated for their time. Can also be a senior resident or nurse.

Nothing worked. My attending looked over and said, “What do you want to do?” Never once, during all of the chaos or his threat to harm, did I think to myself that in order to control this stressful situation someone would have to die. 4/

@Docsocksomaha I love this. So much. I needed to hear it as we are in tantrum season in my house and I’m like “will this ever pass?” Yes, I think so bc my husband and I are a great source of support for our kids. And we shower them with love (and some structure). That’s what I tell myself.😬🙏🏽

@jendegeus Yes, want to highlight cosleeping; all my mom's groups share "safe practices" for co-sleeping. To clarify, cosleeping is not supported by peds/AAP. But since many are going to do it, there are tips that decrease the risk. But again, still not recommended.

Outside the room was a crowd of security, technicians, nurses, & doctors. I felt like a bystander as well as a member of the treating team. We waited outside and discussed a plan. We formed a strategy to calm him. Verbal redirection, assessed his needs, spoke slowly & softly. 3/

Our BWH EM department created a new position, Vice Chair of D&I and an Office of IDEaS (Inclusion, Diversity, Equity, and Social Justice). Our office, led by Dr. Asiku, was provided funding for the internal faculty positions plus a program manager and admin -- who are AMAZING.

Emailed by hospital's generic email address to evaluate a candidate for hire. I have no clue who will read it. One question asked me to disclose if the person has "mental health issues." Why would they ask me? Why would I disclose sensitive information to an unidentified source?

I read a lot of personal statements and I have read many from Black applicants that open with an example of how they were doubted and discredited. At a young age. Doubted at a young age. I'm so sad that many of us have been motivated by a negative comment, myself included

I joined Twitter to share my journey and to meet other women of color MDs. I've seen more WOC MDs in 4 months on Twitter than I have in 4+ years as a practicing physician! Grateful I met 2 bright stars, @emefahloccoh and @LashNolen via tweets and that we can connect in person!

Reading Becoming @MichelleObama for a bookclub @harvardmed "Time ,..., was a gift you gave to other people" Before reading this book, I told my husband I needed senior women of color mentors. This book fills a void. I've never been so deeply mentored through someone's writing.

Grateful to mentor 4 amazing @EMRES_MGHBWH residents as they shared their truth. Often in medicine when women, especially women of color, speak about their experiences we are met with "but that happens to everyone!" No. No it doesn't. Let's stop the denial.

That's a wrap! Goal was 100 but virtually mentored ~180 #medstudents for #match21 . Laughs, stress, sadness, and hope. What a unique process to go through! Will reflect and write. #MedTwitter get some rest if you're doing ERAS interview screening. Tomorrow is a big big day!

I hear from POC residency applicants about layers of isolation from virtual interviews. It reminds me of my own experience applying to programs, sitting around packed tables as the only applicant of color. We hope "Interviewed while Black" characterizes that common experience.

Mentoring Tip: Routine meetings with your mentee can be done with purpose and themes. I tend section them in terms of quarters of year. 1st Quarter: Goals for Year, 2nd Quarter: Meeting Deadlines, 3rd Quarter: Finding Sense of Purpose, 4th Quarter: Reflections on Personal Growth

The short term problem is the lack of diversity on this panel. The larger & much longer term problem? These positions being held below often have no term limits. Without churn, less women and persons of color are able to mobilize upward. This caps our ceiling. 1/

Closing thoughts of the week: Quitting is a skill, not a flaw. Just like any other skill, it only feels bad or off when you do it wrong, when you are learning. Practice leads to comfort. Practice erases the shame. Life is short, spent it doing what you want. And quit the rest.

This is my worry. Much of my success is due to the influence @uche_blackstock had on my career. If she hadn't been there, I might not be here. I hope others see the ripple effect that occurs when even one Black woman doctor is pushed out. It's not just that single person we lose.

The number of Black women leaving academia is terrifying. From @uche_blackstock , @DrBrittaniJ , @DrDennar and @ayshakhoury (to name a few), academic medicine is suffering great losses. Why can’t we be seen and valued *before* being pushed out?

I'm a published author. I struggled to write essays in HS, college, grad school, even as faculty. Never assumed Id be a writer. Never consider writing a skill one can develop. After small lessons via courses, books, rejections - I grew - & felt like I was ready to write a book.

I recognize #DNRTulane gathers people for a mission & lets students, residents have a voice in the situation. But I worry it is unfairly placing burden on (Black) students to solve a problem they did not create. I think we have the wrong hashtag. Call out the responsible parties.

Wow, I can't even imagine what medical school was like for Dr. Oxley. We need buildings and societies named after people with her!! This type of strength and bravery paved the way for people like me.

Despite explicit racism and discrimination while at our institution, Lucy Oxley graduated UC medical school in 1935 and became the first board certified African American woman in IM. After training, she returned to serve walnut hills as part of her decorated career.

Had a great session with 7 med students on interview strategies + how to learn if program is a good match. Discussed how to triage questions appropriately for admin, residents, faculty, APDs/PD, Chairs...Love it. DM if interested. Will do a few more before ERAS opens.

If you had to pick ONE thing to ask to a pregnant, lactating, post-partum, OR parenting person... just ask, "Are you getting enough support?" That's it. It's more important than: sex of baby, gestation week, breastmilk vs formula, yes/no on epidural, desire for more kids, etc..
