Well…what are your thoughts? With all due respect, we all have a role in medicine and this for me is too far. The training and education is not the same, the care won’t be the same. It’s not about deploying a valve or a stent, it’s the decision making and thought process!

PCI isn’t hard! Good, safe and well thought out PCI is hard. Managing complications is hard. Setting realistic expectations is hard. Saying no is hard. Knowing when to stop is hard.
For my non-IC cardiologist, it’s more than wires and stents! 🧠🫀

Why do so many people want to be doctors without putting in the time, work & effort. A nurse or NP will always be a nurse, a PA will always be a physician’s assistant. Both a crucial part of the health care team but not independent medical doctors. Go to MD/DO school, simple!!

Anything beyond that initial “certification” which is flawed in many ways is simply extortion fees and dues to maintain a piece of paper without proof that it improves patient care. People out of touch with medicine should not have a say in our profession.

It should be illegal for pharmacies not dispense a P2Y12 for patients because they cannot afford it without notifying the provider immediately. This could have catastrophic consequences for the patient just because of cost. This is sickening…

Personal & random thought: the auto interpretation of EKG’s should NOT be a thing. Anyone ordering an EKG should be trained and capable of interpreting and understanding the results. Too many physicians rely on an automated interpretation resulting in bad care!

Had an ED charge nurse have the whereabouts to ask me to see a pt before she gave dilt for the exact scenario. Two minutes of actually talking to the patient, I put a prob on him EF 10%! 😓

Not many of us were privileged or came from money to have our parents save to pay for medical school. Many of us did work three jobs and made sure college was paid for but can’t do that with medical school. It’s good to think before you speak and make irrational comments.

Good technique is key and can be achieved with an 18g needle with US and Flouro combo and a regular 0.035 J wire. Visualization of the needle puncturing the artery and mastery of US is the most important step not just putting the probe on the skin!

Absolutely amazing. We had a similar case where I was doing CPR and our very calm and talented interventional attending took a wire, preloaded with a balloon and opened up the ostium of the left main just enough to help stabilize and get MCS onboard. 🙌🏽🙏🏽❤️

Interesting thing I saw today…Two medical students looking at the artworks on the wall in the wards & discussing them. I’ve walked past the same wall 100s of times and not once stopped and noticed it. A good self reflection to perhaps slow down appreciate the surroundings!

I agree it’s certainly more challenging to quantify and you have to be cognizant of it; but it’s obtainable and done on regular basis. This is the reason we have a specialty within medicine as cardiologist to evaluate and interpret these difficult echos.

Busy week but reflecting back it’s been well worth it and a great learning experience! Not to mention I got to wire a 95% in-stent lesion and fix it which felt amazing but not gonna lie I was scared and excited! 💉🩸❤️
