If you sit in awkward silence you're just wasting your time.
Therapy has to be meaningful to the individual. Use whatever motivation is needed in order to facilitate the person. At 8am Jack is walking Jazzie for the first time ever.

Today I'm proud to say that I've been appointed as Special Olympics Global Faculty Advisor for Healthy Athletes.
Mouth breathing has been shown to cause:
· ADHD
· Asthma
· Sleep disorders
· Chronic fatigue
· Allergic reactions
· Facial disfiguration
· Decreased brain activity
What most people don't know is how breathing through the mouth affects the brain.
When a clinician writes:
"didn't engage with physio/non compliant" for people with learning disabilities
what they mean is:
"didn't know how to engage the person, so gave up"
One person who doesn't get enough credit is my wife Liz who, 1 year ago responded with "yeah, do it" when I suggested going going self employed.
Tonight is the inaugural LD physio Christmas Party featuring the ENTIRE TEAM.
Of course, not everyone with Down's Syndrome is the same - I know this. And yes, I used medical language, my target audience is medical professionals, they need to know this stuff and they frequently do not. Save your anger for someone trolling, not trying to make a difference 🙏
Mid Wednesday evening physio confession:
I use Google a lot more than you might think when looking up conditions, terminology etc.
And do you know what?

My job?
At times, it's stressful & almost always busy. You deal with great people sometimes in horrific & unfair situations.
However, for 30 mins this afternoon it was complete & utter satisfaction that we have made a difference to someone's day.

Learning Disability Awareness Week, lots of positives flying around and talk of changing attitudes but not a lot of substance IMO. So, what can you, as a professional in non specialist services learn/do? A thread.🧵
If we went to a healthcare professional and said we were going to sit on the sofa for the next 18 years, we might be encouraged to take some exercise.
But we just accept that this is future for many people with Learning Disabilities, particularly those with PMLD.

I heard AGAIN about an physio told not to worry about team dynamics or worry about "things outside their banding, leave it to the band 7". This hierarchical crap needs to stop.
Chris Nikic IS AN IRONMAN! The first person with Down syndrome to finish an IRONMAN. Chris swam 2.4 miles, biked 112 and ran 26.2. The everyday lives of people with intellectual disabilities are often unecessarily called inspirational. This though, this is inspirational!
My brother Jon, would've been 36 today. I think it's fitting that today I announce the formation of LD Physiotherapy Training & Development Ltd, my new business.
This guy does this from his own house with his kids and volunteers. Making toys that don't exist for people with disabilities so they can have a friend like them.

Someone has a knee problem, they go to a knee specialist.
Someone has a back problem, they go to a spinal specialist.
Someone has a neurological problem, they go to a neurological specialist.
Someone has breathing problems, they go to a respiratory specialist.
Struggling to get a person with dementia to stand to a transfer aid.
Tried standing to a kitchen worktop where there was a bottle of wine, next thing, she is up, has poured herself a glass and his having a sip.
Wine. It works.

Many of you may not know that my late brother won the award for most inspirational sports person in the City of Sheffield back in 2010 (not just disability sports person, sports person). I'm proud to have known this guy and to call him my big brother. I miss you man.

There's a person I work with who has short term memory loss as a result of a brain injury. I once wore odd socks by accident on a visit. It was never forgotten. Now its the best way he remembers me and what we do together.
So here is me, before going on a home visit this morning
Home visit this am, worked with a young man with CP to decorate his family Christmas tree in supported standing for 35mins. He worked really hard as it was meaningful to him. Had his family in tears and he, I and his carer had a blast. I think he did a good job

We usually display pain of loss on anniversaries and memorable occasions. But more often it kicks you during everyday life, memories, things you hear, do and see hit you like an emotional freight train when you least expect it.
You know, I'd love to be a physio who has the time to argue about different types of management of ACL injuries. But I can't, because disabled people aren't even allowed to be f#@*ing represented in society.
Rolling is one of my favourite exercises. Control, coordination and strength all play a part to complete the movement and it can be tiring too.

The reaction I get from people by doing the basics right - listening, explaining, conversing, treating people like people - makes me worry about what on earth healthcare professionals are doing!
P. S I'm not saying I'm great, im saying how low is the bar?!
When manual therapy is shown to not be effective, we can discard that modality.
When exercise is shown to be ineffective we CANNOT forget the innumerable other benefits to people and systems it has simply because it may not help an OA elbow specifically.
Stop it.
"What do you do that makes you different?"
I sometimes go through entire sessions without saying a word if that is what is needed.
I wear odd socks to help a person engage.
I am energetic.
I am calm.
I let people beat me at exercises.
I beat other people at exercises.
If you're a professional and read my posts thinking "ahh, that doesn't apply to me, I don't see people with learning disabilities".
Then you my friend, are part of the problem.
More than 85% of people with acquired blindness experience poor balance and most, falls. Add in one sided weakness and you can see why we are working with Ian on both of those things.

What is my number one tip for building rapport with upset/angry patients?
1, Listen, do not interrupt.
2, validate "yeah that was rubbish, I'm sorry you've had to go through that".
3, "how can you and I work together to be the best outcome".
News flash, I know other people with learning/intellectual disabilities exist.
Tweets threads, despite being quick to read take more time to put together than it may seem, I spent most of yesterday including 2 hour stint at a kids play area to do this one for example.
