Snowplowing is common cause for inability to advance sheath. Might be confused with other issues such as lead calcification. Sometimes can't tell until we pull the lead out. For snowplow size up. For calcium, switch to mechanical. Any tips on how to differentiate?

Traditional morphologies IMO are:
RCC: (+) in 1 with less R wave in V1
R/L: (+) then (-) in 1 with notch on downstroke of V1
LCC: (-) in 1 and “W” pattern in V1

No easy answer but appealing to do this conservatively. Takes effort and experience. Have to debride tissue, remove sutures sleeves, change generator, irrigate like crazy, scrub leads, bury system under the muscle with Tyrx, compartmentalize best you can, then chronic Abx.
