@davidmcswane @DrToddLee @propublica Yes I agree these are complex cases. But I would bear in mind that barbiturates are extremely potent at high doses, so they can be scary to physicians not experienced with them. It's conceivable that physicians who reviewed this case over-estimated the effect of phenobarbital.

@dissengagement If a patient with advanced cancer were to receive 260 mg of phenobarbital and pass on, I would attribute the death to cancer and not medication effect. I don’t think it’s a high enough dose to really accelerate dying.

@tonyver45 @propublica @davidmcswane I think the story is basically accurate and I agree with the author’s general conclusions. It’s really just the phenobarb dosing bit that I disagree with.

I want to discuss an error in a recent article in @propublica by @davidmcswane The article alleges that an oncologist euthanized several patients with phenobarbital. Evidence from the article is shown below. Can you see the error?

Why do I care? Phenobarbital is commonly maligned as an incredibly dangerous drug, leading to underutilization where it could be uniquely helpful (e.g. alcohol withdrawal). But ultimately the dose makes the toxin. High enough doses can absolutely kill someone, but not 260 mg.

throttling tweets that contain links is one more small way that twitter is hostile to journals & FOAMed. any platform that discourages links is self-serving (lookin at you instagram).

Elon Musk effectively admits that posts with links are throttled. If you've been wondering why your posts with links to journalistic articles haven't been getting nearly the engagement you would expect, this is why. I have less and less reason to be here.
