I want to touch upon why I feel Payment Reform is so important for us as radiation oncologists. Over the last 20+ years physician reimbursement has not kept pace with inflation although other aspects of care have done so. One way physician work is evaluated by CMS is via the conversion factor. For 2025 CF = $32.2465; 2204 CF = $33.2875; 2023 RUC CF $33.8872; 1997 CF = $35.7671 If the CF kept up with inflation by some estimates, it would have been $68.58 in 2023. How does it make sense that our work is getting easier and less valuable? Medicine is more complex than even 2 years ago with improved treatments, greater innovation, increasing technologies and more input that we as physicians must incorporate into our practice. I am an advocate of the AMA bill H.R. 2474 that would add a permanent inflation update to the CF however this does not address PE. I recognize our specialty is not alone and that there is strength in finding a common goal across specialties. We know that there are over 300 office-based interventional services where the cost of providing care is less than PFS reimbursement. This issue has plagued our profession for decades and there will not be a quick fix; however, it is vital that we are valued appropriately for the work that we do and the commitment we have made to our patients to provide cutting edge therapies. We need continued education, advocacy and collaboration with our colleagues to address this challenge. ROCR is an excellent option if we can get this passed. I will continue to work collaboratively with like-minded colleagues to address this concern and I am hopeful we will make progress. Please find a link to a paper discussing some of these issues. We have data to support these challenges. I am the Chair of government relations and economics committee of ACRO and have served as the RUC rep for ~6 years and now CPT rep.
