Opinion: vancomycin + cefepime + metronidazole (VCM) is superior empiric therapy than vancomycin + piperacillin/tazobactam (VPT) for *most* indications in the emergency department.

Did you also know that if the order set cannot be updated to best serve your specific institution because it must be updated at the system level and that everyone cannot agree on how to move forward, then there is no point in having an order set?
Rivastigmine is a reasonable alternative to physostigmine for anticholinergic overdose. It has pharmacokinetic advantages like its long half life and slower CNS penetration.

, I was once told while amio and lido are considered equal for VF arrest there is data pointing to better outcomes for lido in ischemic heart disease (CAD, CABG, etc). However, I’ve never found the data. Is anyone familiar with this concept? Can you share some lit?
