A ring enhancing lesion on MRI with internally restricted diffusion is indicative of Brain Abscess. Ddx for ring-enhancing lesions includes abscess, 1’ CNS lymphoma, brain metastasis, glioblastoma etc. Abscess is distinguished via its restricted diffusion on DWI.

Associations between celiac disease and neurologic symptoms include Cerebellar Ataxia and Peripheral Neuropathy, however studies are limited. Nonetheless, when cause of ataxia is unclear, celiac autoantibodies (tTG IgA) should be tested. Ataxia may improve with gluten-free diet.

Topiramate is a carbonic anhydrase inhibitor. It promotes stone formation by reducing urinary citrate excretion and increasing urinary pH. This risk is 2 to 4 times higher than in the general population, higher in men. Adequate hydration is however essential to mitigate the risk.

Sudden-onset headache in a patient with a prior history of idiopathic DVT is concerning for “Cerebral Venous Sinus Thrombosis” While she has a history of migraine, the sudden onset and lack of association with photophobia or phonophobia are suggestive of a more sinister cause.
