Thanks Nigel, even better if we usually use population health segmentation to target SMR to key cohorts ie frail elderly, in community settings AND records joined up so all can see and edit. Good stuff and glad you are doing well.
In older adults 5-10% of hospital admissions are related to adverse drug reactions. Medication reviews are an important aspect of acute care and could prevent future admissions. Also important to communicate clearly any changes with primary care colleagues

Really important message. Long term use of opioids can be dangerous, leading to poorer health outcomes including addiction and increased falls and confusion. Let’s work together to reduce harm. @GeriSoc @PaulaHiett
If you have been taking pain medication for longer than three months, it’s time to book a pain review with a pharmacist or GP. They may stop working and you may suffer adverse effects including addiction if you take them for longer. Read more:

Very yummy Sunday tea with cream cheese or pate
@LoCarbFreshwell a twist on the wonderful “flaxseed roll” recipe to make a low carb “cheese scone”. Mature cheddar instead of the mozzarella cheese and a teaspoon of mustard for a perfect punch. Gluten free as well. Perfect served warm with butter ❤️ @mattrix1979
