@VPrasadMDMPH Because it's an attempt for healthcare corporations to increase the work force and decrease physician pay under the guise of patient access. Almost certainly this will not increase access . Corporate profits will certainly go up

This is spot on. There is enough for us to improve in the field of endoscopy ( polyp resection, biliary stricture dx, therapeutic eus intervention). We don't need to find answers to problems that already have great solutions. Non-Invasive imaging technology is replacing liver biopsy portal pressure evaluation In many cases of suspected portal hypertension.

A very sad statistic revealing another pandemic in the United States. The pandemic of physician burnout, despair, depression and suicide. This must be investigated. I can't but help think this is the result of diminishing physician worth, autonomy and value in lieu of corporate strategy and greed. Physicians are not "providers" Physicians are not "cost centers" Physicians are not "employees"

I have known 7 MDs who committed suicide (6M, 1F) 300-400 doctors kill themselves/year in USA. Suicide attempts 250% higher among ♀ docs & 70% higher among β™‚ We are used to helping, not needing help In USA, call/text 988 for help 24/7 #medx #MedTwitter Please RT

The reflux issue gets brought up probably more than needed. It's almost never a clinical issue. The reality is that both LHM and POEM patients will be on ppi long term. The only group that doesn't need ppi is PD... But outcomes less effective. The reality is that we much more closely have examined reflux rates among poem patients

This is a critical point. The best treatment for achalasia is the one that you can offer with high quality. For me that's poem. If I'm at a center that does high quality LHM but just started doing poem... I'll take the LHM. Similarly if the option is PD vs LHM or POEM (type 2 achalasia). I'll take PD if the experience at that center is low as PD doesn't burn bridges and is relatively uniform among providers

Congratulations to all the residents who matched in GastroπŸŽ‰πŸŽ‰πŸŽ‰πŸ™ŒπŸ½ Few tips from an academic gastro to ponder before you start your fellowship ⭐ Blocking consults or angrily receiving consults is NOT a sign of strength but of laziness. Remember when you are an attending, consults = πŸ’² ⭐ Modern GI is 60 % IBS/functional. Embrace it and learn evidence based strategy. Even as an interventional endoscopist, you need to learn functional GI disorders ⭐ IBD is humbling. Most residents never get exposure and it's a whole new world of nomenclature and data. Take time to read and learn ⭐ Endoscopy is a very rewarding aspect of GI, but like everything it's a skill to be learned. Be humble. No one is a natural. It takes thousands of cases and years to gain proficiency. ⭐ Be prepared to learn anatomy all over again. Your days of calculating CHAD score and FENa are over. But learn what a billroth2, rygb, ryhj, gastrectomy, ileocecal resection look like ⭐ GI is much more than bleeding cases. Like every test, learn what to do when it is positive and when it's negative. Don't sign off simply because the "scope" was negative. That's when the real GI starts 3 years go by real fast. Enjoy every minute of it @AmerGastroAssn @MGH_GI @mghliver @ASGEendoscopy @ANMSociety

Debate on Achalasia Management: PD vs. POEM | VuMedi via @vumedi Had a great time with this debate on PD vs POEM with the great #PeterDraganov. This was tough, but anyone convinced that PD has a role in 2024???

@esaagar Actually this is far more nuanced and any effort at simply reducing calories through will power will fail. As my colleague and obesity medicine expert #LeeKaplanMD has elegantly stated. "You're not obese because you over eat... You over eat because you are obese" @GeiselMed

Well stated. I have no inherent bias against epic. The problem is 1. User interface is clunky and not intuitive 2. Inbasket has none of the normal functionality of any email service (outlook, Gmail) 3. Simple tasks require an enormous number of clicks (12 clicks for pre-op orders) 4. Price tag is crazy considering the product 5. Enormous amount of documentation bloat that doesn't help Compare epic to @amazon or @gmail User experience. It's night and day. The difference is that the customer is not physicians and nurses, but hospital administration,and hence, no Accountability
