With his theory of the soul, experience creating cursed wombs, and access to Sukuna's body, I believe that Kenjaku fused part of Sukuna's body with an unborn Yuji.
Attempting to create a vessel that has Sukuna's body, at it's peak, to house a weakened soul of Sukuna.

In the last panel of 227, Gojo shrunk his barrier around Sukuna's. Taking away Sukuna's advantage of sure hit range, eliminating Sukuna's ability to attack from outside the barrier. Putting them at even footing and back to a normal battle of technique.
