Last day as GP today, made special even as a Locum by the lovely people I worked with and the patients I saw. I feel very blessed to have been a GP for the last 30 years. It really is an amazing job.

So 35 years after being added I have now been voluntarily erased from the medical register. I miss my team and my patients, but not the work. I have been lucky to work with amazing people and have fantastic patients. I feel blessed. Now I need to move on to the next adventure. 🥰

Our 19 year old just asked is it right that people can can now come to the Lakes but I can’t go and see my grandparents who live down the road. I hope our villages are not swamped by day trippers & 2nd home owners or we will be overwhelmed and community stress levels will rise.

English GP contract announcement once again shows no rural proofing. The access provisions will be difficult in urban areas and near impossible to deliver in rural areas. Will our patients travel 20+ miles for an appointment with a practice they don’t know ?

Sadly another example of why we are where we are and the partners sadly will hand over our contract at 6.30 on 30 December. We have always put our patients needs at the forefront of our services. We don’t need a letter from our ICB to tell us that we need to do that.
Every single day secondary care breaches its hospital contract and they are rightly provided with support. Meanwhile, primary care gets these emails.
Our masters are probably in Qatar watching the world cup whilst we are logged in on a Sunday wishing we were with our families.

Thought we were meant to be moving services out of hospital into the community. Less headline grabbing though as that requires multiple smaller investments. 90% of nhs work is done in the community so if we announce £13bn hospital funding can we have £117bn community investment ?

So some really good news today from our area. The deanery are struggling to find placements for GPs in training as the numbers are oversubscribed for the first time in a long while. And even better we are going to be helping with training from August ! (Fingers crossed)

Today is another sad day as we say goodbye to Julie Coates who retires at lunchtime today. Julie has been reception, admin , manager , tea & coffee provider , biscuit consumer and an all round hero keeping the show on the road. Thank you Julie and enjoy retirement. ❤️

Running a service from Windermere to oxenholme on a single track line needs one train, one driver and one guard for the 2 journeys each hour. The train runs one way than returns . Surely in this country we have at least one spare train that Northern can borrow
Just been on the phone with Northern. They told me that no trains will be running on the Lakes Line from Monday. This is disgraceful. If they can't run the service, then it should be turned over to someone who can.

Please stay off the high fells. Our hospitals are full, our rescue teams are volunteers many in key services and we need them in the NHS etc. This wave is impacting not just “old”people - forget the fells until spring now - it’s just not worth risking an accident
Local mountain rescue teams have been reminding people only to venture out if they have the right kit, and the knowledge and experience to keep themselves safe.

Son left at 7.30 for breakfast shift at hotel this morning having got in from last nights evening shift at 1.30. Same again for the next week although he finishes at 6 on Xmas day. Bear a thought for all our hospitality workers who work ridiculous hours over this period. Tip well

Today our medical student came to Hawkshead. We didn’t have space to teach them in our old surgery. Busy rural day full of lots of unplanned activity. He loved the building and loved it more when we explained everything in it was paid for by community fundraising.

Also spare a thought for the forgotten people working tomorrow - those in the hotel & hospitality sector . Many away from home for the first time & all will be working long hours over the Christmas period. Tip well if you get good service as they are giving up their time for you

Would recommend to any GPs in training (or urban based policy makers) to get an understanding of the challenges of farming in our area. So important to understand the issues of the rural community when you are embedded in it

Hawkshead is open ! Beautiful valley in between Windermere and Coniston Water. Road via A591 at Ambleside . One of the best commutes you can do ! Or via Newby Bridge. Walks, food .... and a great community (they funded our surgery so biased !)

Must say enjoyed Christmas Eve with our Italian, Spanish, Hungarian & Slovakian friends sharing stories & traditions . All work hard here and have lived here for a long time. So sad that as a country we are giving our the wrong messages about being welcome here.

Yesterday we held a meeting between practices around Lake Windermere to discuss contingency planning. We never had access to the technology to do this online until this crisis. Saved a huge amount of travel time as we have said for ages. Progress in difficult time’s.

It was like of meeting of the clans as patients & staff travelled from across the villages of our large area ! A modern day equivalent of the Lakeland shows hopefully not to repeated after 2021. Great spirit, fantastic volunteers, great organisation & a big car park !!

This makes very scary reading. I have been one of those GPs described in this document and I know it’s not sustainable in the long term. We need action now before the situation becomes irreversible.
Looking for some lunchtime reading-we have just published a report on the financial distress our practices are in.Almost 60% of practices reported financial stress either in terms of reduced income/rising costs&the inability to afford to recruit staff.

Immediate concern ahead is staff needing to self isolate. Already been down a GP and ANP in recent weeks. They can work from home but in a limited way. Would be good if commissioners accepted we will be impacted & protect resource so we can get on with whatever is ahead.

Jane leaves us on Friday. Amazing to realise that since 1990 our area has only had 3 GP’s - Jane, Kaye & Chris. In an era of increasing digital technology often the importance of human contact can be overlooked . Knowing your patients & your patients families is key to being a GP

Parking update - the 4 spaces provided for patients are being used for non surgery access reasons . Due to this you will now need to leave your Reg number in reception as spot checks will now be performed to verify that spaces are being used for genuine surgery access.

Today is one of those days when you really get the feeling the NHS wants general practice to fail. It just puts so many barriers in place & thinks it can dump costs onto practices and cut their funding at the same time. Utterly depressing at the moment.

Given the quick adjustment to online meetings you do wonder what the purpose is of HS2 is now. Travel will be mainly for domestic purposes in the future so a more leisurely pace would be fine. Seems a very sensible idea.
