@ashishkjha Even if the only question was acute mortality, how many death do we expect in a “bad flu” year? How many COVID deaths this year? Right.

@SolidEvidence Thinking about the increasingly higher “baseline” wastewater after subsequent waves. If a % are chronically shedding (some in very high amounts) could that account for this? Do the metagenomics of the wastewater suggest some of the high concentration are derivative extinct VOC?

@SolidEvidence I would add that the FDA is also supporting an “antiviral” that is a viral mutagen, and in many cases it is being given preferentially to patients with immune compromise because of fewer drug interactions. In my view, literal real word, uncontrolled, GOF.

@SolidEvidence Anecdotally, more clinical disease in immune compromised persons but is that due to waning immunity/poor revised vaccine up take? Hard to say. Still some scary outcomes even in “fully vaccinated” IC persons, and for reasons we have discussed, I hate molnupravir.

Would also be interesting to see analysis of the work hours of all the “quality and safety” “modules” forced on heath care practitioners (+ the administrative time to be sure they are done every year). For all that cost (including 🔥out) shouldn’t there have to be some benefit?
New @JAMA_current study shows that Johns Hopkins Hospital spent $5 million and >100,000 person hours in 1 year to report 162 quality metrics. Worth it? #medtwitter
