@rbarbosa91 You’re not wrong but i can’t think of too many operations where laparoscopy offers an advantage over the robot. Certainly in my world (liver/pancreas/surg onc), 🤖 is superior. There’s plenty of data, it’s no longer debatable.

Unless OS or PFS are improved, I would argue it’s not a risk worth taking. This reminds me of this editorial on the enigma of regional nodes in melanoma and cancer in general.

Makes sense. I did have a case where the mesenteric disease was unresectable but the patient had recurrent bowel obstructions from bulky nodes adjacent to the primary.. I did resect and it helped but that’s only one case so I can’t generalize. We used to stop when we felt there’s significant risk of bowel devascularization when I was in training at Iowa.
