I guess at a certain point it becomes a question of how deep the rabbit hole goes.
To fix primary care it would be helpful to fix poverty, substandard housing, broken communities, broken families, the food industry, capitalism.......

Thanks. I'm in the lucky position of not *needing* the work (but it would have been more convenient in terms of commute and working hours). But I know lots of GPs who are feeling it at the moment.

The PA in our practice is excellent.
I'm a little torn on the whole thing.
She manages patients appropriately and discusses all of them at the end of the clinic. We get slots blocked out to supervise her, so it actually makes our clinics easier.
And we know she won't leave.

I suspect the patient had good levels of health literacy and the time/money/motivation to make these changes.
How should we approach a patient in whom these factors are missing?

I find this such a fascinating area.
On the one hand shouldn't we be free to make occasional unhealthy choices?
But on the other, are we really free in the choices we make in relation to these things when billions are spent finding the best ways to make us buy them?

I really hate the people who insist that the patient signs the form before they can leave. By all means have a chat, assess their capacity, understand that our priorities aren't always in alignment and let them go with a bit of safety netting advice.

I came to this post from a repost in which a geriatrician robustly defended the term being used in A&E.
I think it has utility but clearly care needs to be exercised.

Her kids have just started at the local school and she owns nearby. But you're right it's no guarantee. However, we know for sure that our F2s will leave.
Our F2s aren't used for service provision. They get 4 patients per session.

I think it's just a phrase that people have latched on to, like "patient denies".
It's mainly down to a lack of training in the art of letter writing.
Personally, I'd rather that secondary care ask me to review a lot of these things. I probably know them better than any other

Well, ok you got me there. We can't *know*. But we do know the exact date that our F2s will leave.

It's not abysmal. I could well imagine an F1 with low confidence, who partied through their pre-clinical years doing this.
At least they know that 60% is less than 100% so credit where credit is due.
