This (along with some curation) slowly created a very useful set of codified knowledge in a much more effective and focused way than "writing documentation".

Previous company where I worked had a policy that if someone explained something to you (via voice/chat/email/whatever), then you had a responsibility to document the explanation properly where it would be useful to others.

The trend of using chat for questions is also plaguing a number of organisations that I've been involved in (with the same outcome).
