The Great Barrington Declaration was absolutely wrong. No vaccine or immunity is completely protective, and in fact we saw people with vaccination and natural immunity get really sick during the first years of the pandemic. Claiming a comparison to Semmelweis is deeply unserious

@jlsalinas7 @MartenHawkins I actually wrote about this a while back in Contagion, but I think it is going to be a decent choice in UTI and maybe a few other indications. The big reason trials are mediocre I think has to do more with the trial definitions of success than actual clinical efficacy shown

@TxID_Fellows @PergamIC @swinndong @KatesOlivia @DocWoc71 @EmilyBlumbergMD We have a ton of resources here at @UMassChan, so much so that it’s taking some time to get adjusted and access. But have had talks with mentors and stats people aplenty, and there are local grant programs you can apply to as well

Now that we’re on the topic: Isolated fever is a POOR predictor of bacteremia. Shaking chills are better indicators. HOWEVER, you also need to consider the pre-test probability based on the syndrome in question.

@mmPharmD @PharmDFarmD How could a patient with recurrent C diff need more than 80 tabs of Vancomycin in 180 days? It defies all logic to consider they might need more!

@mmPharmD Dealing with an arbitrary dispense amount in a time period limitation for a patient with a clear need for more than that amount…. I’m a boiling cauldron of rage right now
