I prefer 24 fr tract size which offers faster clearance without rise in pressure. Ultrasound guidance if needed for initial puncture. Ofcourse I don’t have experience with suction in RIRS

I have converted to PCNL in such cases where I have seen matrix stone during RIRS.

@GuidoGiusti We are also seeing lot of ureteric strictures and also ureteric avulsions at PUJ following endourologic management of upper ureteric stones. Even our guidelines consider old procedures like ureterolithotomy not a one of the primary option for large ureteric stones.

@dravinashsingh We also used to push back the stone in the urethra into bladder and do CLT. But in the given case the stone was impacted in pendular urethra and doing laser ablation in that location is generally difficult in such a large stone. Hence we opted this approach.

Sometimes #stones get impacted in #urethra and better to offer open surgical removal. Endoscopic treatment might cause more injury and can lead to urethral stricture. @SocietyGURS #urethroplasty @sanjaybkulkarni @drjoshi_pankaj @Bhav_Tez @SKSingh2k2
