We are REALLY feeling the strain. We don’t have enough ICU beds. Many patients are being intubated as soon as they show up to the Emergency Room. We are doing our best to take care of them on regular floor beds and in the ER itself. But it is HARD. .

So much interest in Lp(a)!!. Had an awesome time presenting our study investigating Lp(a) variability among patients from the OCEAN(a)-DOSE trial. CVi 10% . Max abs change of 135 nmol/L. Guidelines recommend testing Lp(a) once in a pt’s lifetime. Time to rethink? #AHA2023

Many #COVID19 patients have been intubated for longer than I would have expected. But there have also been uplifting moments. Some of the patients I have cared for have been extubated and are recovering.

Looks like we should now be using cognitive behavioral therapy in Cardiology. New study in JACC reports a 56% reduction in healthcare consumption and improvements in quality of life among patients with atrial fib who underwent CBT. Fascinating!.

Diabetes remains a major risk factor!. Published now in @CircAHA, we found a higher risk of adverse CV events in pts with DM and LM dz. Moreover, compared with CABG, PCI resulted in no difference in the risk of death, but there were key differences for nonfatal events.

A novel role for PCSK9 inhibitors? . At #AHA2024, we showed that early initiation of evolocumab was associated with fewer arterial aneurysms @TIMIStudyGroup @AndreZimerman.

In pts with LM disease, those with ACS had higher rates of early CV death and MI. Risk of 5-yr all-cause death was similar with PCI vs. CABG in those with and without ACS; tradeoffs for nonfatal events with PCI vs. CABG were consistent regardless of ACS.

In our hospital, several #COVID19 units are being converted back to regular floors to allow care for patients with a variety of different health concerns. After being on solely #COVID19 units for the last few months, this transition definitely feels surreal!.

Today, there is no effective pharmacotherapy for aortic stenosis. In this @CircAHA study, @AeronSmallMD and colleagues found several novel genomic regions associated with the condition.
Calcific aortic stenosis is common and has no effective medical therapy. We performed a multi-ancestry GWAS of 14,451 CAS cases in the Million Veteran Program, identifying 6 novel genomic regions and doubling the known GWS loci in CAS. (1/8)

The hardest part of this has been seeing the heartbreak and emotional pain that many patients' families experience. Sharing that their loved one is getting sicker or not going to make it after days (or in some cases, weeks) on a ventilator. it is utterly devastating.
