I was honored to perform a live case during #TogetherVT 2023 in @ikem Prague moderated by Katja Zeppenfeld and Frank Marchlinski. Two distinct VTs were inducible in a young male with ARVC. After endo-epi ablation both were rendered noninducible. #VTablation #EPeeps

Very nice data. PF may be a game changer for VT ablation.
PFA may be superior to RFA for VT ablation as it eliminates myocardium separated from the catheter by collagen/fat. Our new paper also unlocked a major limitation of RFA in scar, showing lower max intra-myocardial temperature compared with healthy tissue.

@EladAnter In my experience, the anterior line is better choice if the anterior wall is extensively scarred (due to previous ablation attempt or spontaneously). I have seen patients with isolated appendages, when lateral MI line was done and scarring on anterior wall naturally progressed.

@MasatoF0728 @EladAnter Hi Masato! Thanks. No proximal RBBB was created. In my experience this is not required (and does not help). The problem arises at the periphery pf RB. I still wander what makes a healthy midlde aged male suddenly acquire this terrible condition.

@EladAnter 42 years old otherwise healthy man. Best prematurity was just below the moderator band. Ablation there abolished the ectopy and no VT recurrence for 3M now. Epicardial mapping did not show any structural changes. Neither did further extensive cardiac evaluation. Strange disease..

Yes, the patient had situs inversus.The ICE images were not remarkable (we go with ICE across the septum to the LSPV and its relationship to LAA was similar as in other patients).The difference was that all the rotation maneuvers had to be done in opposite direction.
