I’m a huge fan of the NHS. The staff and clinicians are world leading. But the processes and facilities are creaking. A colleague of mine spent 2 nights sleeping in a chair in A&E last week before a bed was available. It needs resource and we need to pay. You will need it.

This concentrates largely on those aspects potentially amenable to preventative intervention (i.e. cervical dysfunction and premature uterine contractility), as well as strategies to improve outcomes for infants born prematurely.

There is a reduction in mean T2*, prior to spon preterm birth <32weeks showing less perfusion of the thymuses and the potential for advanced MRI techniques to interrogate the fetal immune changes prior to preterm birth in vivo ⁦⁦@lifecourse_KCL⁩

There is a need for cervical scanning to be available for symptomatic women as prediction is the key to allow targetted interventions and improved outcomes. We urgently need training programmes while alternative biomarkers are evaluated in the UK.
