Imagine being 5’0 petite female nurse working graveyards so you go shopping at midnight on your night off at the 24 hr Kroger’s. Someone is keeping close tabs on you in the store and then tries to follow you to your car.
But what do I know, it only happened to my wife.
Congrats to our 32 year old who survived covid. Trach. Peg. Can’t walk yet and still on a vent. But he’s in the 99% who survived.
Vaccination likely would have prevented all of this.

South Korea has 7300 cases and 54 deaths. A .7% covid mortality rate. It’s lower because they are testing tens of thousands of people.
Admit the testing here was botched.
So last year 37,000 Americans died from the common Flu. It averages between 27,000 and 70,000 per year. Nothing is shut down, life & the economy go on. At this moment there are 546 confirmed cases of CoronaVirus, with 22 deaths. Think about that!

Texas provider here. I’ve asked uterus-having-patients their last period start date for my entire 30 year long career. I’m asking as a professional about something that could affect patient care. Not as an agent of the state.
My nursing colleagues are holding the hand of a 50ish year young unvaccinated previously healthy lady who is saturating in the 20’s and is end of life with covid. She can still respond to voice.

If an officer won’t do their job and is willing to compromise a case because they don’t like your outside opinions then they should turn in the badge and go do something else.
One of our 30 year old covid patients was asked about code status. He said it wouldn’t come to that. That was then.
Dude’s been on ecmo now for two weeks. Vtach from cardiomyopathy. CRRT. A vaccine could have prevented this.
Fam: Update please..
Us. We are off pressors. Still on vent and CRRT. Still dying from untreatable end stage metastatic cancer with multisystem organ failure. Her prognosis is grave. We still recommend comfort measures only.

Rather than being beholden to their provider’s schedules.
Just admitted a very old person presenting from a nursing home. Ams. PNA. Covid positive. Unvaccinated because fam thought it was too new.
VERY angry when I refused to prescribe ivermectin. I told them there’s no evidence. I was loudly told YOURE WRONG.

For some folks the anxiety of “not knowing” is more disabling or distressing than knowing bad news.
They can process and grieve in private instead of having audience.
They can formulate their questions before return to the office.
Or they can choose to wait to find out.
Did some old school cupped hand chest slapping percussive postural drainage tonight. Got a lot of crud up and dropped fio2 from 50 to 35%.

And. While being fully vaccinated, I am now covid PCR positive.
I almost certainly caught it at work in the ER. I was complacent and just wearing surgical masks unless strongly symptomatic or positive patient.
I’m very mildly symptomatic. Symptoms started yesterday.
My Hospitalist colleague admitted a 30 year old with covid hypoxia. She was in ivermectin prescribed by a clinic/urgent care doc. That clinic provider told her don’t g to the hospital because she’s die.
Admit sats were 70%
Rn: can you come look at bed 11? He’s looking kind of janky....
Me: janky? Is that initial or subsequent encounter?
Rn: it’s acute on chronic jankiness.
Unpopular view. I started as an ADN.. A paramedic to ADN transition student.
Ultimately adding the BSN did nothing to increase my nursing knowledge or ability, only my career mobility.
Had a very rough night tonight in cvicu. A postop ruptured his PA and was exsanguinating. Massive transfusion. Surgeon came from home and opened the chest at bedside. Then to OR. He lived.
We just did our jobs, Well.
But I felt like I was just along for the ride.
I’ve got 6 covid vent patients on my unit between ages of 35-50. All unvaccinated. Most were productive and healthy before covid. I’m not sure any of them will leave the hospital alive.
All of this was preventable.
Houston’s mayor cancelled the city convention center from hosting Texas GOP convention over covid concerns. Deep Red Montgomery County to the north steps up and says “we will host you, come on”......

Trying to walk out the front door of the ER and L&D was there to get a laboring mom at triage. All of a sudden moms pants got lumpy and started crying.
Helluva entrance to the world, kiddo. Baby doing well. Left a bit of a mess.

Another avoidable MRI accident caused by non MRI personnel putting a patient in the magnet room without the tech being part of the process.
This is why MRI techs in some places would LOCK the scanner/magnet room door to prevent unauthorized access.
Just got an intubated pt who was tubed hours ago, roc'd and given NO sedation in ER til he arrived in my unit.
I'm what they would call a bit peeved right now. (Aka fucking livid).
This is unacceptable practice. And as a former ER RN see an avoidable medical error.
People. If you are a nurse/RT/Tech/doc &you are faced with leaving your job over PPE, there is no honor or benefit in resigning. None.
If it’s suddenly going to be your last day, make them fire you. In writing. Tape it. Video it.
Don’t threaten/blackmail but don’t QUIT
And once again, because the family will not accept reality, I have intubated an octogenarian in multisystem failure from metastatic cancer. She will invariably die. Part of me wants my next tattoo to be "DNR" on my sternum...

I caught covid despite the vaccine. Probably from a patient at work. I’m a high risk for death or severe covid disease.
I had mild disease because of the vaccine and early administration of monoclonals.
I didn’t use hcq or ivermectin.
Blood pressure doesn’t equal perfusion.
Oxygenation doesn’t equal ventilation.
Treat the patient, not the monitor.
Actually touch the patient when assessing.
Listen to your patient.
Slow is smooth, smooth is fast.
Learn your craft. Master your craft.
I got two emails this week at work. One to tell everyone they are getting a $100 holiday bonus. One to the doctors telling them to choose between an Apple Watch and ipad6 as their holiday gift. I’m pretty sure I wasn’t supposed to see the second one.

I used to get in trouble in elementary school for correcting the teachers. They finally called mom.
First question she asked: was I correct?
Them: “well, yes”.
Saw only 30 patients for covid testing today.
12 positives. 40% positivity rate.
Most had 11/26 as their date of exposure to someone who later became covid positive.
If only we had known in advance that visiting family on a holiday was a bad idea. 🤷♂️🤷♂️
As a veteran RN and APRN, with multiple populations and roles, and years of healthcare before nursing, I will shout from the rooftops that you have no business trying to be an advanced practice RN before you are an experienced RN. Your patients deserve that.
Today I moonlit as a Hospitalist.
Two of my 18 patients needed to vent about their prolonged hospitalizations. So. I sat there and listened to them vent their anger and frustration.

I had a nurse at a houston free standing ER do the same when I asked a patient their pronouns. It was reflective of the whole shops culture.
I’m just an ally and I’m tired. I can’t imagine what it’s like for you.
While my wife was going to the bank to put the deposit down on our dream home, the dogs got out while she was gone. Our 100 pound grey bully Gus had a bad day on the street in front of our current land, and has died.

I used to do my trauma patients hair in ICU.. pick the twigs and blood out... shampoo... use lotion for a conditioner and detangle.. then blowdry with medical air.. I was fortunate when the assignment would allow me to do such things..
So. While vacationing at Disney I got a call. Our Emergency General Surgery service wants to interview me for their open NP slot.

I've got an atypically large number of young folks on my service dying of metastatic cancer.. who are full code.. who've exhausted all options... And want "everything". And they will die very soon anyways.
