Psychiatric Mental Health Nurse Practitioner| Learner| Left-handed| Umi Says | NJ-DMV-NY|I like to give my perspective, and I like to learn about other's too.
I've never met anyone that agrees with every single thing I agree with or rejects everything that I reject. Nor have you. We know this. You can't push a square peg through a round hole. But you can teach a square about your circle.
Unpopular opinion: We are being too nice and PC and it's getting people killed. Some people barely passed bio, but we are supposed to pretend that their opinion on covid and vaccines is just as important as an epidemiologist. /1
I am not a mechanic. If you let me fix your brakes, you will crash into a wall. For this reason, I will never give car advice, and I will never attempt to fix your car, because it will put you in danger. /2
I couldn't tell you the number of times that a patient experiencing psychosis has made a claim where, on the surface, it sounded outrageous but turned out to be completely true.
Listening to our patients is important, even when it's noisy.
But when people are so blatantly wrong about vaccines (some of whom are healthcare professionals), my patience to be nice and respectful is wearing thin.
People are dying due to stupidity and the acceptance of it. /4
There is a wave of people that think that they know something, and they don't. They talk among eachother, and the ball of bad information gets bigger.
Maybe it's a failure of our public health system, maybe it's a failure of some of our academic institutions. I don't know./3
To shut your mouth and listen. Conspiracy theorists, YouTube, inability to interpret data, viral posts, people that studied to pass instead of to learn, family members ect..
If you are spreading false information I would like to issue you all a strong, shut the fuck up./6
There isn't enough reverence for experience and expertise. Certain things take time to learn and master. Everyone is not an expert in all things. Most of us are ignorant to most things.
Doesn't mean we don't have to ability to learn, but if you don't know something, it's ok /5
In the past week, three nurses America were killed while on the job.
Most of the nurses that I know have been assaulted at some point while on the job.
It bothers me when people attribute my size to what makes me a great psych nurse.
"It must be easy for you to intimidate aggressive patients."
Why the hell would I want to intimidate a patient?!
I hear shit like that regularly. /1
@GrantB911
This type of stuff I have a problem with.
He's got a huge platform and he's speaking to Black people and telling them nonsense.
He could've just kept it 💯 and said he's frustrated that the pandemic messed with his 💰. I get it, artists make a lot of their money from touring.
A lot of men are violent towards women. A lot.
Anyone who tries to deny that is not moving authentically.
If you can't understand why a women would want to protect themselves from men, asking the women in your lives about their experiences may prove to be eye opening.
@KimberlyNFoster
I think part of the discomfort comes from the way we've conceptualized what marriage is. Our ideas around marriage are shaped by a lot of messages that are inaccurate. Creates false expectations. Marriage can be beautiful, as long as the ones in it make it what they* want to be.
During my first year as a nurse, I often didn't take breaks.
Thought it was a badge of honor.
Busting my ass in a CPEP.
But then, I grew.
Understood how I was depleting myself.
Now, every break is taken. All PTO is used. Even some non PTO is used.
They won't break my soul.
Today a Pt walked past me while rapping, looked at me and stated that I have, "no bars".
Knowing my skillsets transcend nursing, I told Pt that I've got bars for days.
Pt said let's battle
We battled.
Pt lost said battle.
But it was the 1st smile I've seen on pt all day🙂
Many Americans are disconnected from the ills of war. They didn't have to endure it, so there is an extreme lack of understanding for how ugly it is.
This pandemic has some similarities. Many people are just living their lives, while a war goes on in our hospitals. /1
Just met a Black man psych NP in the hospital today.
I told him I was on the same path and how hard the grind is while Im working and schooling.
The way this brother hyped me up!
Made me feel like I was playing football again listening to a pregame speech!
I love us.
The shift isn't over until it's over.
I remember one shift as a new nurse, looking at the time.
It was 6:45.
I thought I was home free.
I was wrong. I was so so wrong.
Yea yo.
I've done the calculations.
When I'm charging, I am asked a question or spoken to at least once every 17.2 seconds, for all 12.5 hrs of my shift.
@CandiceElam7
Too many people feel like that should speak on everything and everyone has an audience. Freedom of speech is great, but aren't there some provisions that are in there that limit harm?
Please, thank you, you're welcome.
A smile, eye contact, sincerity.
Love and kindness.
Self care because the work can be taxing.
But we're not trying to intimidate over here. We're trying to help people through things the that we can't always see. /4
Dealing with potentially violent situations is a regular part of the work, but verbal de-escalation skills are key.
Also, humanizing people who may be struggling with something is important. IMO, if you can't do that, or learn to do it, you don't belong on the ward. /3
Psychiatric Urgent Care should be more prevalent than it is.
Every mental health crisis/problem does not require all that comes with the inpatient experience.
The inpatient experience can sometimes be a deterent to many that want care.
And don't get me wrong, I'm aware of how my stature may impact an interaction, but I lead with my ears.
I often get assigned patients that are labeled "aggressive" and many times those "aggressive" pts end up w/tears in their eyes becasue I simply gave them space and an 👂🏿. /2
@angela_rye
The critique was less about you and more about your affiliation with Charlemagne. It's a valid critique. You ignoring it and taking it personally does nothing to address the sentiment that many feel. I get yall are friends so do what you do, but many feel the same way.
When I meet a pt for the first time
I usually say, "Good morning, my name is Omarr and I am going to be your nurse today."
When you meet a pt or family for the first time, how do yall introduce yourselves?
Do yall introduce yourselves?
@NicoleLewisLCSW
They are not. These though, are amazing. $20, great sound, long battery life and doesnt fall out of my ear. Forgot them in my pocket once, case and all, and washed them. Came out working perfectly. If I lose a pair, doest hurt as much as the Airpods.
@EKing719
Glad that you were able to advocate for your child and I'm happy that she is doing OK doc. Many of the people in these comments need a thorough lesson on patient centered care.
Spit at
Punched at
Kicked at
Cursed out
All just today.
I'm in the psych ED but this experience is universal to nurses in many settings.
The amount of abuse we encounter contrasts so heavily with the amount of care we provide for people. /1
Everyone knows about the school to prison pipeline.
But I think there's a trauma to prison pipeline.
A neglect to prison pipeline.
An abuse to prison pipeline.
A coping to prison pipeline.
Systems have undervalued nurses for quite some time.
Now that the script has flipped they want to introduce legislation to cap the pay of nurses.
Are they trying to cap the salaries of hospital admin?
The all love capitalism until it doesn't work in their favor.

@car_snacks
I would say that there should be space for reality orientation. But being thorough in our assessments can inform us more on what the reality actually is.
You know, the first two years of the pandemic really took something from me.
I can't fully put my finger on it, but I'm just not the same person that I used to be.
Even when I am doing many of the same things as I did before, it just feels...different.
So many workers have lost their lives. So many have witnessed more death than they ever had. I worry about the collective aftermath for our healthcare workers.
We leave veterans out to dry in this country. The same will be true of our healthcare workers. /3
Part of me wishes I discovered nursing earlier. But then again, my experiences before entering the field shape so much of my present and future practice. I like that I have more direction.
#NurseTwitter
That hero shit sounds a lot like "Thank you for your service".
Hollow.
I'm a psychiatric nurse, so I know my experience is different. But I listen to the stories of my colleagues, and it's heartbreaking. /2
The usual sequence of events.
1. Unfortunate event happens at facility.
2. It's determined that it's nursing's responsibility.
3. Additional tasks are added to nurses day to prevent future occurrence.
4. No tasks taken away from nurses.
/1
A patient being upset that they are in the psychiatric ward is not the same as agitation from psychosis.
Them adamantly not wanting to be there is a reasonable response to treatment that is sometimes forced with conditions that may be less than ideal.
Use some discernment.
Last time I checked, men and women have equal access to guns...
Yet whenever I read about these heartbreaking stories..
there's a man at the front end of the barrel...
My guys...what the fuck are we doing?
No should be required to put themselves in harms way in order to perform a service.
It's a choice.
It's autonomy.
We all have it.
And we should respect each other's boundaries.
People setting boundaries are not personal attacks.
But people tend to personalize things.
There are plenty of reasons that many plan on leaving the profession, and this is one of them.
No one wants to go to work and be subjected to violence.
It's traumatizing.
And for three nurses this past week, the result was the loss of their lives.
Nurses often see people when they are they are in a vulnerable place.
But it seems like this job of caring for people also leaves us in a vulnerable place.
Therapy is for everyone, but partly because we live in a society that normalizes traumatizing people.
Collectively, we're not doing life right.
Somethings gotta give.
36 y/o and I'm finishing my first semester of NP school. Sometimes I get down on myself for how much more I have to go.
I wonder if I am missing out on anything? Is life passing me by as school consumes so much time? /1
What does it mean when psychosis manifests as racism.
It's happened twice this week in two different hospitals.
Not looking for the "sorry that happened to you". I'm good. Trust me.
But im curious.
What do you think it means when someone who is psychotic says racist things?
@DrRamblings
It's so dangerous. People are dying like it's nothing and so many couldn't give two 💩s.
Some of it is willfull but I think sometimes there's a failure in how we deliver information. Whether it be how we teach people or how we dispell false info, we're not doing it will enough.
So much about losing a parent that they don't tell you
You don't just lose them
You also lose the world that they created for you
It's been 8 years and the rush of pain is the same
I'm ok
But im thinking about all of you who have lost a parent too
Shit fucking sucks
One of the most heartbreaking parts of my job as a psychiatric nurse is seeing some patients who, over time, decompensate and have a new baseline that is less functional than before.
When I knew them 5 years ago, and the "them" that I knew is no longer here.
@becomingRNBSN
I tend to agree though. Everyone in healthcare does not have all the facts. None of us have ALL the facts. The hope is that people try to be as informed as possible before speaking and giving out advice.
Who our teachers are matters. Our experiences reg who teaches us can impact our perceptions of reality.
Who knows science?
Who's qualified to speak on scientific things?
Who can become a scientist?
This concept transcends much more.
We get subtle messages that inform us
Psych units should be tiered to the type of acuity/chief complaint a patient has.
Everyone under a single umbrella isn't a good look.
The needs of patients are too vast and the presentation of one pt can have an impact on other patients on a particular unit.
A man was murdered on Monday on an NYC subway train.
On video..
In front of a crowd of people..
The man's name was Jordan Neely
He was homeless and had a hx of mental illness.
The man who murdered him, is a 24 year old ex marine.
He was not charged with anything..
I've said this before.
Last time I checked, men & women have equal access to guns.
Yet whenever mass shootings happen, the common denominator seems to be men.
Can we get real about the unhealthy ways some men deal with emotions?
This goes beyond mass shootings.
#Boulder
I'm so tired of remembering passwords, resetting them, not using the past 6 passwords, adding a symbol, doing a dance, a back flip, and pretending that every password for every thing is that damn different.
Just scan my retina and get it over with.
Black people are regularly misunderstood by mental health providers...
Cultural competency is important and can greatly impact the trajectory of care.
We need more providers that understand their patients.
“Patient referred to writer as ‘fam’.
Patient is clearly delusional as I am not part of patient’s family.

Hear me out. Pack your bags, plan to be gone for a week. Get your spray from home depot. Get in your car, start the engine, roll down the window (preferably at night). Shoot your shots, and roll out. J/k🤣 I don't know what I'm talking about. Just burn it all down 🤣
10 years today.
I miss you.
I dream about you.
I know that you're with me.
I'm pushing,
I'm pushing with the energy that you've blessed me with.
Watch me work mommy.
I here to make make you proud.
8 semesters down, 2 to f****** goooo!!! 🥳
Me, a ΨNP?!
Sometimes, I still can't believe that I am where I am...
But then...I remember how hard I worked to get here...
"Alexa, play "Dreams and Nightmares". 😎
@TraumaSoapBoxes
For once I would like admin to shadow me and we do everything that is asked of us during a shift. I want them to see it up close, look me in the eye and tell me it's feasible.
@TheBlondeRN
I'll never forget the day a full gallon of water was thrown at me and hit me on the side of my face. Saw it coming out the corner of my eye but I wasn't quick enough.
Later that day, same pt was choking and I gave that pt the heimlich
Pt lived.
My face was hurting for days
@shesinscrubs
I would create public health focused non clinical track in every nursing school. It would be focused on education, health promotion and the prevention of illness. Take all of what you learned and give it to the people to help keep them healthy.
@KaylaAncrum
I'm used to getting the "covert", plain clothes ones that follow you around the store and pretend that they are shopping. I usually just start following them if I have time.
I like your idea though. Noted.
You know what did get built though?
Prisons. Lots and lots of f*cking prisons.
Right now prisons and jails hold more people with serious mental illness than hospitals do.
We crminalize mental illness and put ppl in prison--an environment that causes further decompensation. /4
