How can the NHS stand for health when Beth’s physical and mental health needs were unmet in hospital during the years she was locked up. She doubled in size, putting her in the obese category, she hadn’t visited a dentist, she was medicated but still had depression and anxiety.

If the Oliver McGowan training is mandatory, then why aren’t 100% of staff doing the training! 🤬 Also why are they only starting Tier 2 face to face training this year?!
Online learning is not good enough as there’s no means to know if staff have done it

My daughter, Beth was Autistic and locked up for 3 years with no therapy or discharge. She lost her life in there January this year aged 21. She went in at 18 and lost all her adult years and skills 💔😢

A public inquiry into the death and neglect of Autistic young people, wrongly diagnosed as Personality Disorder is paramount. Autistic lives matter. Hospitals are dangerous for them. So many are losing their lives and under 30 years old. This needs to END!

Had a meeting regarding Beth’s Patient safety incident investigation from TEWV before it goes to the Directors panel. Lessons are not learned and there is no preventative action to prevent future deaths so there will be more to come.
They just don’t learn 🤬🤬🤬

TEWV claim they no longer follow the Protocol for the reduction of harm associated with suicidal behaviour, deliberate self harm and its treatment (for people with a diagnosis of borderline personality disorder and related conditions), which ended in 2020 🤨🤔

Her bp levels were abnormal but not looked into, she had a seizure whilst on leave with me which was not explored.
Beths so called care was neglectful. Beths death was preventable

To be told they don’t take things of patients as it’s not common practice and using strongs is not dignified, yet stripping, restraining and injecting is ok apparently 🤷♀️🤦♀️

How things have changed from the great NHS that once had the reputation of being caring for the most vulnerable and staff depicted as Angels, to now where the NHS are at crisis due to negligence, becoming a business and that patients =🤑

Yet they still tolerate patients attempting suicide without consequences, simply because if they want to then that’s on them.
Materials are not taken away and they allow this to go on, knowing what the end result will be

Why are there no IMHAs (advocacy) Autism? This is definitely a need. Beth had an advocate in hospital to represent her in meetings, but Beth didn’t tell her things as she was too clinical

‘Least restrictive’ only applies when it suits. Beths items of harm couldn’t be removed, YET restraining her was the biggest restriction, causing psychological and physical trauma.

Beth was locked up on a section 3 for 3 years with no exit plan. Reason for being in hospital was ‘bed and board’ No therapy, no goals, no hope.
She deserved better 😭😔

They didn’t remove risk because of fear of making Beth worse so there were no preventative measures

Beth was 21, no discharge in 3 year, lost her life whilst inpatient as her risk was not managed. This was in January 2024. The PSII report is still not complete! How many more deaths have happened whilst waiting. This is part of the problem and bigger picture 🤷♀️😢

I have friends in the NHS, who are Angels and want to make a difference, but the system itself doesn’t work anymore. Staff are overworked, are under appreciated, staff are often off sick with stress, there’s a reliance on using agency staff.

Beth was kept for almost 2 years on PICU wards on male corridors with male trauma. This is not okay. Drop downs couldn’t manage Beth’s risks but her risk was not managed as items of risk were never taken away out of fear of challenging behaviour! Lack of boundaries and safety 😔

Thank you. I’ve never known such a toxic culture before. Even the report is given to the panel but I Beth’s parent don’t get to attend and speak up and question things. This is a huge part of the problem. Family voices and communication

Imagine someone reading this PBSP and it’s advising to rush Beth and the last point to try and rationalise when distressed! This is not Autistic aware and a sham of a PBSP. It’s a list and is not clear to pick up and support!

Next steps will be waiting for an inquest date but I know that there is already a backlog 😔

TEWV not only failed to meet Beth’s mental health but also her Autism and physical health . She had not seen a dentist in the 3 years, she doubled in weight and BMI putting her in a dangerous category. She had no therapy /psychology. They neglected her 😔😭

Staff were to come to see Beth for a week and then her staff to go with her for a week to stay. This did not happen. First night staff sickness so couldn’t attend.

Maybe look at TEWVs website ‘Our big conversation’ which will give you a raw and honest insight into the publics opinions. Yet it is still under ‘requires improvement’, deaths are happening and there is a public outcry. Nobody is concerned about the concerns-it’s the culture

How do they classify ‘severe autism’ 🤦♀️ According to labels, Beth was Autistic -Asperger’s aka ‘high functioning’ . However, she wasn’t functioning in other way; such as she couldn’t make decisions, lacked organisation and had poor social skills. It’s harmful and dangerous xx
