Actually in the cutscene where Kakyoin executes Dio, Dio strikes Kakyoin but doesn't kill him immediately. This plane scene shows that Dio's last attack was actually a mortal blow and also falls in line with Araki's lore that fate is unavoidable and can only be delayed.
