Space Shuttle Orbiter has 3 SSME hydrolox engines. Before LOX gets into the main combustion chamber (MCC), it has to go through a series of posts that inject it into the MCC. Hot hydrogen passes in between those posts and heats up the oxygen.

SSME nozzle is regeneratively cooled, meaning that it's cooled by hydrogen that then continues to the MCC. The nozzle has 1,080 long narrow cooling tubes. It is believed that losing 5 adjacent tubes would cause a burn-through and a chain reaction that would destroy the engine.

The computer commanded a throttle down on the center engine to reduce the - in reality nominal - chamber pressure. The oxygen (and hydrogen) consumption was therefore reduced and the center engine miraculously made up for the right engine which was using too much oxygen.

That meant that the mixture ratio in the MCC changed. There was more oxygen than there should be, which resulted in higher temperatures. The turbines were 100°C hotter than they should be, the temperature margin was 200°C.

In the end, Columbia ran out of LOX only 0.15 seconds earlier than planned. The oxygen shortfall was only 183kg (it would have been ~10x larger hadn't the AC1 Bus failed) and Columbia's velocity after MECO was only 4.5m/s lower than desired. This was easily corrected with OMS.
