Teaching without jokes is like iftar without samboosa.
On a more serious note, teaching isn’t stand-up comedy, but an 8 hour teaching session needs some humor to keep us all going.
All the ink in our world would run dry before I could express how much I adore you. For I didn’t fall in love with you, I walked into love with you. I’ll love you till the end of time and beyond! ❤️

For now, I'll be covering Biostats for STEP 1 and will branch off to other topics with time. 🤓

🌟 SUMMARY:
✅ Avoid using single data points to manage CHF.
✅ Bicarb changes are NOT associated with clinical decongestion.
✅ Bicarb ↑ is iatrogenic 2/2 diuretic-induced acidification.
✅ Hx, exam, and POCUS remain vital in the care of a patient with🫀failure.

If there’s one thing this year has taught me, it’s that there is a hidden beauty to the way we fall; a hidden blessing is present with every failure.
Sometimes, God destroys our plans before our plans destroy us.

Congrats to all my fellow juniors on finishing their academic year! 🙏🏼🎉 It was a tough one, but it’s finally over through your hard work!

Fun fact about our

If you guess every single question, the negative marking actually doesn’t balance equally and you gain a net average of 0.25 points per 4 questions.
In other words, statistically speaking, you should never pick the “I don’t know option”.
In case you’re on twitter instead of studying, allow me to supplement your break with a video of me breaking down some beloved scenes from Grey’s Anatomy.

Great to be back at this year’s ACP on the luscious Stanford Lawn with good company! It was a pleasure to meet some old friends and share our projects together. 📊
Well, it’s complicated, so let’s learn together.
Contraction alkalosis was a term describing the phenomenon when decreased extracellular fluid volume prompts activation of the RAAS resulting in increased bicarbonate concentration (as a byproduct of aldosterone secretion).

I am immensely grateful to those who left an impact on me, those who engraved within me a deep desire to be a more compassionate, reflective, and curious doctor. It’s easy to forget that those are the qualities that differentiate a good physician from a great one.
Here’s an adorable (yet deadly) thrombus in the left atrium that’s just a little a bit too big to pass through the mitral valve.

In the summer of 2018, I was accepted at Mayo Clinic in Rochester, MN as a research trainee via my university. This opportunity came to me because of my cGPA, research experience, extracurricular activities, and teaching experience. It was sweet victory among defeats.

Throughout all of medical school, I’ve made mistakes, had failures, and experienced defeat. Not once could I blame it on anyone but myself. These were some of the most important events that shaped who I am today.
But two days ago, I was a failure, and I don’t know why.

It’s also explained here. Diuretic-induced acidification of the distal tubules leads to increased production of serum bicarbonate, and this is why we see the increase.
We cause it. Then, we think patients are getting better because of it.

Because the Step 3 📝 isn’t required to apply for residency, you can’t find that much info about it 🧐

You can’t imagine how painful it feels to not get the visa. Take it a step further: how painful it is to get it then not go because others believe they understand your situation better.
Thank God I have the family and friends I do to help me through this tough time.
🧵6/x
Still, I persevered because I knew my happy ending is somewhere waiting for me. Ten months is what it took to comfortably pass 240+ on this exam.
I have a score I am proud of. But most importantly, I beat my limits. The little voice telling me “it’s hopeless” was silent.
🧵9/x
However, just when all the pieces fell in the right places, my visa was held on processing. It crushed me to know that I failed, even more so knowing I couldn’t do anything about it.
It didn’t matter how many emails I sent nor how many calls I made.
I failed.
🧵4/x
This is your kind reminder that UWorld is a learning tool and NOT an assessment tool.
I would also like to remind you that there is an option to turn OFF the % next to the answer choices.
I know my self-worth should not be tied to the visa, but I also know that getting this far was not a mere stroke of luck.
I got here because of persistence and determination, and if there’s one thing you can take from all of this, it is to fight till the last bitter end.
🧵16/x
Thank you for reading! I hope it was somewhat useful and fun to read.

In my cardiology rotation, I full out went 🇫🇷 and pronounced it the RIGHT way when asked for a spot dx on an ECG.
My colleagues were ready to mock me till the end of the rotation. It was still worth it.
Deep down, I still believe this is in some way good for me. However, I’d be lying to say I didn’t feel frustrated. Hell, frustrated doesn’t even express how I feel. It’s a huge understatement.
Writing emails back to the hospital informing them about this tore my heart.
🧵14/x
It’s messages like this that make my day. It’s hard knowing whether or not the videos I make do help, but even if it helps just one person, I’ll never regret the time I spent making them.
There is still a chance to get the visa. I ask of you to keep me in your prayers.
I have until around May 22nd to get it. If you can help me by retweeting this so that it could in some way help me, I’d forever feel grateful.
Please share any info that could help me.

Best of luck to everyone participating in the match!
Irrespective of the result, I really can’t stress enough how much we’re incredibly proud of our seniors for taking this important step!

Boy oh boy how wrong I was. Despite getting accepted for an elective at a hospital, my visa has again been put on processing.
Everything that could go right did until the very last moment. It was a harsh reminder of what I was so very afraid of.
🧵11/x
I offered to come learn how to become a better physician to possibly serve the country’s citizens in good faith in the future.
But, my fear was true. Regardless of my capabilities, my victories, and my achievements, none of it mattered when it came to my visa.
🧵12/x
Thus, it also concludes that a CHANGE in bicarbonate was not associated with CLINICAL decongestion and this is the 🔑
As with all things in CHF, we never use a single marker. Nothing beats a good physical examination and (as
@Thind888
likes to remind us) POCUS.

In came my STEP 2 CK, and the same mentality gave me yet again a score I am proud of. It finally came to the part of my career I needed to finalize: my clinical electives.
In came all my awful memories with visas, but surely there has to be a happy ending?
Right?
