We have found that patients with neurologic manifestations of #COVID19 have inflammatory cytokines in their CSF, at levels approaching that of CAR T cell neurotoxicity. What’s more, this can be detected weeks after the acute infection. Role for steroids?

@YochaiReemMD Good question! I suspect the CSF cytokine elevation is more closely linked to systemic CRS severity than to cancer, based on our matched CoV- controls. Selecting for milder patients and smaller cytokine panels (we used a 92-marker assay) might also explain reporting differences.

@winstoncb Unclear. Drawing from our experience managing CAR T cell neurotoxicity, tocilizumab (an IL-6 receptor blocker) improves systemic cytokine release but does NOT improve neurotoxicity; in fact it worsens neurologic outcomes. Most biologics also don't cross the blood brain barrier.

@ahandvanish It would be highly valuable to learn if this finding also applies to #LongCOVID! There certainly seems to be some symptom overlap.

@ahandvanish All had at least a mild dysexecutive syndrome or "brain fog." A subset experienced additional specific neurologic symptoms: severe prolonged delirium following intubation, limbic encephalitis, headaches, seizures, etc.
