I’ve seen this. Trached laryngectomy patient with “no pulse” tighty grabbing the wrists of CPR provider. I suggested that despite “no palpable pulse”, that he was pRobably OK now and they could stop.

Certainly “less hoops” in a single practice, but my point is that no person (patient) has a right to repeatedly anyone, nor is a healthcare provider obligated to keep treating a patient…you have to document and report.

We operated on someone with terrible ischemic vascular disease (with ulcer) with 2 fevers...3 days later started to cough....you guessed it. COVID positive. Everyone surprised and now all isolated, to be tested. **Assume now all patients may b positive. Be careful out there!

Traditionally used for “too sick for open TAAA” or “bad lungs”, we found that good-risk Pts do REALLY well, suggesting it’s a viable alternative vs open

A lot of the work regarding colonization + infection with catheters was done by Dr. Maki. There’s logarithmic rise after 3 days which led to most policies of moving peripheral IVs. Arterial lines tend to stay in longer + can’t be just put elsewhere.

There were so many randomized control trials showing the benefits of CT over physical exam I am shocked that this even comes up anymore. I was one of the people thought physical exam was enough, but level one data consistently shows otherwise!!

Example of how to get skin to reach a little farther! I will try with the next fasciotomy that I can’t quite close and hate to have to skin graft them!
I use “pie crusting” mostly for fasciotomy wounds, but it was described by
@FractureDoc
in 2007 for the feet as well. I use it in the setting of tight dorsal foot wounds if I do a midfoot or Lisfranc type injury to increase my comfort level with the skin. They heal great.
