The glitch happened, again, on his end of the game.
Skylar does NOT use any tools or cheat engine, or anything of the sort, he was playing through Arcade Mode, and once he finished the mode Kazuya appeared on the screen on his end of the game.

They did state that the individual, or people? who worked on designing Reina created both Kazumi and Josie, so that would make sense as to why Kazumi's face was probably used as a base to 'change' it to be original as best as they could've made it for Reina.
