@chris_malkin4 @TribelessSapien We had a case like this recently. Very similar after withdrawal of the pigtail and with check injection. We believe it was laceration of the sinus by calcium spike on leaflet. Was there hostile calcium on the leaflet? Can send you the images.

Great option for these heart failure patients who have frequent admissions, poor QoL and resistant to diuretics.
