From Struggles to Success: A Journey of Surgical Excellence and Giving Back to Society As a humble surgeon from Telangana, my journey to becoming an onco-surgeon and later a robotic surgeon has been marked by perseverance, dedication, and divine guidance. With only five seats available across India, the path to specialization in onco-surgery was challenging, leading me to train at premier institutions like Adyar and Kidwai. My pursuit of excellence took me further to the USA, where I honed my skills in robotic surgery. Reflecting on this journey, three significant contributions stand out, which I am grateful to have achieved with God’s help: 1. Pioneering Rural Cancer Surgery In 2000, I initiated rural cancer surgery services in Nizamabad and Karimnagar, a mission that continues today at Chalmeda Cancer Hospital. This initiative, aimed at providing care for underprivileged Arogyashree patients, underscores my commitment to making advanced cancer care accessible to rural communities. Ensuring that those in remote areas receive quality cancer treatment has been a central part of my mission. 2. Launching the ONLINE MASTER CLASS During the challenging times of the COVID-19 pandemic, I launched the ONLINE MASTER CLASS under the aegis of Telangana ASI. This initiative, started on April 9th, 2020, with the esteemed Prof. Vijay Kumar Sir, was a pioneering effort to continue surgical education through online platforms. The Facebook links serve as a testament to the impact and reach of these classes, offering proof of the valuable knowledge shared during those sessions. It was a step towards ensuring that learning and skill development continued uninterrupted despite global challenges. 3. The ‘Teach 10, They Will Teach 10’ Initiative My passion for teaching has driven me to visit the maximum number of institutions across India, spreading knowledge of robotic surgery. The vision behind “Teach 10, They Will Teach 10” is simple yet powerful: to create a ripple effect of learning, ensuring that surgical expertise is passed on and expanded exponentially. The map of my travels stands as a testament to this mission, highlighting the extensive reach of my educational efforts. This initiative aims to create a network of skilled surgeons who continue to impart their knowledge, thus enhancing the overall quality of surgical care. The Role of Social Media While social media can be overwhelming, it serves a crucial role in documenting and providing timestamps for every event and session I have conducted over the years. It stands as an undeniable record of my journey and contributions to the field of surgery. Through these platforms, the progress and impact of my work can be tracked and shared, ensuring transparency and inspiration for others. Gratitude and Mission I extend my heartfelt gratitude to my parents, friends, dedicated team, and God Almighty for the strength and support that have enabled me to live a life dedicated to rural outreach, robotic surgery teaching, and continuous learning through both online and offline master classes. Together, we continue to make a difference in the lives of many. Rural cancer surgery and robotic surgery have remained my mission, driving me to serve, teach, and give back to society.

Another Great Day of Robotic Surgery Mentorship in Coimbatore Another fulfilling day of robotic surgery mentorship in association with #WeAreIntuitive at G. Kuppuswamy Naidu Memorial Hospital, Coimbatore, training Dr. Arul, Dr. Praveen Ravishankaran, and Dr. Venkatesh, alongside Intuitive facilitator Mr. Shanker. We continue to work toward our goal: Teach 10, and they will teach another 10—ensuring that robotic surgical techniques remain simple, reproducible, and accessible. Case 1: Robotic Left Lower Lung Lobectomy We started at 8:30 AM, but as soon as we inserted the camera, we encountered extensive adhesions due to the patient’s history of tuberculosis and smoking. After carefully releasing the adhesions, we docked the robot, only to discover that there was no fissure between the lobes—making what is usually considered an easier surgery significantly more challenging. Step by step, we meticulously removed the cancerous left lower lobe, sequentially stapling the lower pulmonary artery, vein, and bronchus. Given the fissureless nature of the lung, we used robotic SureForm Smart Staplers, which are invaluable in lung surgery due to superior clamping, ease of angulation, and enhanced security for vessels. Despite the complexities, we successfully completed the procedure. A special mention to the anesthesia team, Dr. Praveen for his excellent surgical skills, and his dedicated assistants who contributed to the case’s success. Case 2: Complex Duodenal Tumor Resection The second case involved a tumor critically located in the first part of the duodenum, near the junction of the first and second part—a challenging site due to its proximity to the bile ducts and hepatic artery. Complete removal with adequate margins was essential while ensuring the pancreas and biliary tract remained unharmed. Additionally, the patient had a high BMI (>110 kg), making the procedure even more demanding. We performed a totally robotic gastrectomy with intracorporeal anastomosis to restore the food pathway—achieving a minimally invasive outcome without opening the abdomen. A Race Against Time (and a Flight!) As we wrapped up the case, I realized my last flight at 10:20 PM was at risk. But as always, the patient comes first. We completed the procedure with full dedication, only to find out that the flight was delayed—allowing me just enough time to race to the airport and catch it. Perhaps, divine intervention played a role! A big thanks to the entire oncology team at GKNM Hospital and the Intuitive team for their unwavering support in advancing robotic surgery. Roses and Thorns: The Dual Nature of Life and Relationships Life is often compared to a rose plant—a timeless representation of the coexistence of beauty and pain, joy and suffering, success and struggle. A rose plant has more thorns than flowers, yet we call it a rose plant, not a thorn plant, because the fragrance and beauty of the rose make us overlook the thorns. Similarly, in human relationships and personal growth, virtues and flaws exist together, but if our deeds bring goodness to others, our imperfections may be forgiven or forgotten. The Rose and the Thorn: A Symbol of Relationships Relationships, like the intertwining vines of a rose bush, embody both love and challenges. In friendships, family bonds, and professional associations, we experience moments of happiness, but also misunderstandings, disagreements, and trials. A strong relationship is not one devoid of thorns but one that acknowledges them, navigates through them, and still cherishes the beauty of the connection. Spirituality and philosophy teach us that suffering and joy are intertwined, just as the rose and its thorns grow together. True wisdom lies not in wishing for a thornless rose but in understanding how to handle the thorns without losing appreciation for the rose. The Struggle of Those Who Rise from Adversity For those who come from humble beginnings—without privilege, without support—the journey is never just about roses. It is filled with thorns meant to suppress, discourage, and wound us. Struggles in education, career, society, and even within our own minds act as these thorns, making the path to success painful. But the key to victory is not to avoid the thorns but to learn from them. We must: •Taste the bite of the thorns and let the pain strengthen us. •Watch them carefully to learn the dangers and avoid unnecessary suffering. •Face them head-on with resilience and determination. •Walk over them when necessary, trampling difficulties to reach our ultimate goal. No one achieves greatness without facing hardships. The greater the goal, the sharper the thorns. But those who understand this duality—who embrace the struggle instead of fearing it—become unstoppable. The Mahabharata: A Tale of Roses and Thorns The Mahabharata is filled with examples of this principle. One of the most striking is the life of Karna. Born with immense potential, Karna was constantly surrounded by thorns—his unknown lineage, his rejection from society, and the curse of his guru. Yet, he possessed the fragrance of unwavering generosity, valor, and loyalty. Despite the pain life gave him, Karna’s virtues made even his enemies admire him. However, his association with Duryodhana, his one major flaw, became the thorn that ultimately led to his downfall. Similarly, Draupadi’s life was full of roses and thorns. A woman of exceptional strength, she was the queen of the greatest warriors, yet she suffered immense humiliation. Her story teaches us that pain and greatness coexist, and only those who endure suffering with dignity emerge victorious. Even Krishna, the divine strategist, had to navigate the thorns of war, deceit, and loss to establish dharma. He never removed all thorns but helped others walk over them with wisdom. Embracing the Thorns to Reach the Rose The truth of life is that roses and thorns will always coexist. Struggles will never disappear completely, but they should not deter us from reaching our passion. Those who understand this balance—who accept difficulties as part of the journey—become unstoppable. When we recognize that roses and thorns are together, always, we no longer fear obstacles. Instead, we rise above them, knowing that beyond the pain lies the fragrance of success. So, let the world throw thorns your way. Walk over them, rise above them, and claim your rose.

Surgery is not just about skill—it is about focus, endurance, and an unshakable mental state. This is where meditation becomes a game-changer. Beyond the operating room, meditation is a universal tool that can help everyone—irrespective of religion, belief, or background—to lead a healthier, clearer, and more powerful life. The ancient wisdom found in the Bhagavad Gita, the Ramayana, and the Mahabharata emphasizes the power of meditation and inner control in achieving clarity, resilience, and strength. The Science of Meditation: How It Transforms the Body & Mind Medical research has proven that meditation rewires the brain, strengthens immunity, and improves decision-making. Some of its key benefits include: 1. A Clear, Focused Mind •Meditation helps declutter mental noise, making decision-making faster and more precise. •For a surgeon, this means greater focus before and during surgery, leading to better outcomes for patients. •In daily life, this translates to calmness, better problem-solving, and emotional stability. 2. Strengthening Immunity & Overall Health •Meditation lowers cortisol (the stress hormone), which in turn strengthens the immune system. •It reduces inflammation and helps fight ailments, including heart disease, high blood pressure, and even chronic pain. 3. Enhancing Emotional & Mental Resilience •Meditation makes us mentally stronger, helping us deal with uncertainty, setbacks, and challenges. •It teaches us to respond, not react—a skill that is invaluable in surgery, business, relationships, and life. 4. Cooling the Mind Before Surgery •A few moments of deep meditation before surgery allows a surgeon to enter a flow state—a state of complete absorption, awareness, and precision. •It helps maintain cool-headedness, ensuring steady hands, sharp judgment, and unwavering confidence in the most critical moments. Ancient Wisdom: Meditation in the Mahabharata, Bhagavad Gita, and Ramayana Bhagavad Gita: The Power of Inner Control In the Bhagavad Gita, Lord Krishna tells Arjuna: “A person who has control over the mind is tranquil in heat and cold, in pleasure and pain, and in honor and dishonor.” (Bhagavad Gita 6.7) Arjuna was filled with doubt, fear, and confusion before the great war of Kurukshetra. Lord Krishna taught him Dhyana Yoga (the path of meditation and self-control) to silence his fears and make decisive, fearless choices. Similarly, when we meditate, we clear doubts and fears, making our decisions strong, quick, and right—whether in surgery, leadership, or personal life. Mahabharata: Meditation & Mental Strength Dronacharya, the master of archery, trained his students to focus with one-pointed concentration. When he asked his disciples what they saw before shooting at a bird’s eye, Arjuna replied, “I see only the eye of the bird.” This was the power of meditative focus—eliminating distractions and achieving absolute precision. Whether wielding a scalpel or making life’s crucial decisions, this state of deep focus is what meditation gives us. Ramayana: The Strength of a Calm Mind When Hanuman was searching for Sita in Lanka, he meditated deeply before making key decisions. Even in the face of enemies, he remained calm and wise, knowing exactly when to attack and when to retreat. This teaches us that in every battle—whether in an operating room, a business, or life itself—meditation helps us stay composed, wise, and strategic. Life Lessons from the Gym: A Blueprint for Harmony and Growth The gym is more than just a place to build physical strength; it is a microcosm of life itself. Every day, we step into a space where we interact with both familiar and unfamiliar faces, yet we remain neutral, respectful, and often greet each other with a smile. If only we could carry this same approach into our daily lives, the world would be a better place. Observing and Adapting When we enter the gym, we take a bird’s-eye view, scanning for available equipment. We don’t push or demand; we patiently wait or move on to another exercise. This teaches us an essential lesson—adaptability and patience. Life, too, doesn’t always give us what we want immediately. Instead of forcing situations, we should learn to wait, adjust, and focus on what we can do in the present moment. Respect and Consideration If someone is using a machine, we don’t rush them; we respect their space and time. Similarly, in life, we should respect others’ journeys and not impose our own timelines or expectations on them. If someone struggles with weights, we instinctively offer help without hesitation—just as we should extend kindness and support in everyday interactions. Teamwork and Unconditional Support Sometimes, we train together, spotting each other, correcting form, and encouraging progress. We do this without expecting anything in return, simply out of camaraderie. Imagine if we applied this in life—helping others without hidden agendas, fostering a sense of unity. Overcoming Struggles Together Occasionally, someone may experience a muscle cramp or strain. Without being asked, we step in, offering advice on relief techniques. This mirrors how we should respond to challenges in life—when we see someone struggling emotionally or mentally, a small act of kindness can make a huge difference. A Bhagavad Gita Perspective This philosophy aligns beautifully with the teachings of the Bhagavad Gita. Lord Krishna advises Arjuna: “Karmanye vadhikaraste, ma phaleshu kadachana” (Bhagavad Gita 2.47) “You have the right to perform your duty, but you are not entitled to the fruits of your actions.” Just like in the gym, where we focus on training without immediate expectations of results, in life, we should focus on doing our duty with sincerity and without attachment to outcomes. Similarly, another verse states: “Samatvam yoga uchyate” (Bhagavad Gita 2.48) “Equanimity is yoga.” In the gym, we remain neutral—no envy, no arrogance, no judgment. We celebrate each other’s progress, understanding that everyone is on their own journey. If we could embody this same neutrality and sportsmanship in life, we would foster a world of mutual respect and harmony. Conclusion The gym isn’t just about lifting weights—it’s about lifting each other. It’s about patience, respect, self-improvement, and community. If we apply these principles in life—waiting our turn, supporting without expectation, helping without hesitation—we can create a more understanding and compassionate world. Honesty and Transparency in Cancer Treatment Cancer treatment is a journey filled with complexities, emotions, and difficult decisions. As surgeons, our responsibility is not just to operate but to guide patients and their families with honesty, transparency, and evidence-based recommendations. Yesterday, I encountered multiple clinical scenarios that reinforced this principle: 1️⃣ Esophageal Cancer Case A patient from South India, diagnosed with middle-third esophageal adenocarcinoma, was prepared for robotic esophagectomy. His children, who are doctors themselves, had traveled from outside India with the expectation of immediate surgery. However, the correct approach for his case was FLOT chemotherapy followed by surgery. Though initially disappointed, they accepted our evidence-based plan after I encouraged them to cross-check with experts worldwide. 2️⃣ Lung Cancer in a High-Risk Cardiac Patient A patient with early-stage lung cancer (Stage 1) had a history of heart surgery, and the cardiologist assessed a high surgical risk. We explained all possible complications—ventilation issues, heart attack risk, and the alternative of Stereotactic Radiosurgery (SRS). Despite the risks, the patient chose robotic surgery. With God’s grace, the procedure was successful, and he was discharged yesterday. 3️⃣ A Young Girl’s Ovarian Cancer Scare A young girl came to us distressed with a diagnosis of serous carcinoma of the ovary. A laparoscopic ovarian cystectomy had already been performed elsewhere, with the tumor removed piecemeal. To ensure accuracy, we reviewed her histopathology slides, and fortunately, the final report confirmed a benign lesion. Instead of unnecessary treatment, we reassured her and advised observation. Her relief was immeasurable. 4️⃣ Understanding Targeted Therapy & Immunotherapy Another patient believed that targeted therapy or immunotherapy would work like laser treatment, directly attacking the tumor. However, cancer treatment isn’t that straightforward. We explained that targeted therapy requires specific molecular markers (NGS/IHC testing) to determine effectiveness and that these treatments work systemically, not like physical laser beams. The Core Values of Surgical Practice These cases reaffirm the values that must guide us: ✅ Honesty & Transparency – Patients and their families deserve to know the stage of the disease, treatment options, cure rates, and possible complications. Sugarcoating or withholding information does not help in the long run. ✅ Evidence-Based Treatment – Cancer treatment must be guided by science, not emotions. Every decision should be backed by research and best practices. If a new treatment is considered, patients should be fully informed about its potential benefits and risks. ✅ Acknowledging & Managing Complications – No surgeon can claim to have never faced complications. If someone does, they are either lying or haven’t operated enough. Complications happen; what matters is how we tackle them and how well we communicate with patients and their families. ✅ Empathy & Counseling – Every cancer patient is going through immense physical and emotional distress. Taking time to counsel them, answer their concerns, and offer support is just as crucial as performing surgery itself. At times, we may lose patients who seek alternative treatments or those who want a different approach. We may not always make the most money. But practicing with honesty and integrity gives us the greatest satisfaction—knowing that we did the right thing for our patients. The event, organized under the esteemed leadership of Dr. Ramchandra Naidu sir, Dr. S. V. Rammohan Reddy, Dr. Hari Haran (President, ASI Kurnool City Chapter), and many other eminent surgeons of Kurnool, brought together a diverse group of surgical experts and oncology colleagues, including Dr. Ravindra Babu, Dr. Beesanna, and many more. The presence of enthusiastic young postgraduates further enriched the discussions, ensuring the next generation of surgeons is well-equipped for the future of robotic surgery. Robotic Surgery in Telugu States: From Passion to Purpose During my session, I shared the journey of robotic surgery in the Telugu states—how we pursued it madly and passionately, overcoming challenges, and how we have given back to society by mentoring across India. From Karimnagar to Nizamabad, from Warangal to Kurnool, our mission has remained clear: to take robotic surgery beyond urban centers and into rural regions where its impact can be transformative. Training, Mentorship, and Expanding Reach Our philosophy is simple: Reach Rural, Teach Ten, Who Will Teach Another 10. Every surgeon trained in robotic techniques creates a ripple effect, extending the reach of cutting-edge cancer treatment to underserved populations. The enthusiasm of IMA and ASI Kurnool doctors reinforced the importance of collaboration in achieving this goal. Future Perspectives & Emerging Trends The discussions extended beyond current robotic applications to emerging trends in robotic-assisted oncology. With AI integration, next-generation robotic platforms, and hybrid surgical approaches, the future is bright for precision oncology. The role of mentorship and skill development remains critical in ensuring these advancements are accessible and effectively utilized. In Robotic Surgery, We Must Relearn: 🔄 Instrument movement – Instead of planar motion, robotic instruments offer 7 degrees of freedom, mimicking the human wrist. 🔄 Camera control – Unlike traditional laparoscopic cameras, robotic cameras move in multiple directions, like a car’s steering wheel, offering better visualization. 🔄 Magnification techniques – With 5x to 10x zoom, we can now see and operate on small, intricate structures with unparalleled clarity. 🔄 Suturing skills – Robotic instruments allow for intuitive, natural hand movements, eliminating the counterintuitive nature of laparoscopy. The key is not just embracing technology but retraining our hands and minds to take full advantage of its potential. The Evolution Cycle: Learn, Unlearn, Relearn—And Teach 📌 Learning keeps us growing. 📌 Unlearning keeps us relevant. 📌 Relearning keeps us ahead. The journey of a surgeon mirrors the life of a lobster, a creature that thrives by shedding its shell, embracing discomfort, and growing stronger. The Lobster Analogy: Growth Through Discomfort A lobster does not grow within the confines of its hard shell. As it outgrows its current form, it must break free, shedding its rigid armor to develop a new, larger one. This process is not easy—it is a phase of vulnerability, discomfort, and risk. Yet, without it, the lobster would stagnate. As surgeons, we face similar moments. Medicine is constantly evolving, and the skills that once defined excellence may become obsolete. To remain at the forefront, we must be willing to adapt, unlearn, and relearn—to step out of the comfort of what we know and embrace new technologies, techniques, and perspectives. Adaptation in Surgical Practice From open surgeries to laparoscopy, from laparoscopy to robotics, and now towards AI-assisted surgeries, the field has seen monumental shifts. A surgeon who clings to old methods without evolving risks becoming outdated. The best surgeons are those who continuously challenge themselves, learning new procedures, refining techniques, and adopting innovations that enhance patient care. Mastering Upper GI Endoscopy and Colonoscopy has transformed rural surgical practice, allowing remote surgeons to diagnose and manage conditions effectively. Similarly, robotic surgery is revolutionizing outcomes in oncology, gastrointestinal, urology, and gynecology. Each step forward requires letting go of old shells and embracing the new. Mahabharata and the Art of Rebirth The Mahabharata offers profound wisdom on transformation. One striking example is the story of Arjuna and his exile. When stripped of his royal comforts, Arjuna did not lament his situation; instead, he used the time to retrain, evolve, and return stronger. Under Lord Shiva, he gained the Pashupatastra; under Indra, he mastered celestial weapons. His growth was not just about acquiring new skills—it was about unlearning arrogance, adapting to challenges, and preparing for the battles ahead. As surgeons, we, too, must embrace continuous learning. The unlearning-relearning cycle is what ensures we remain competent, compassionate, and cutting-edge in patient care. The Future: Strength Through Change To be relevant in surgery, we must be like the lobster: •Recognize when growth is needed—stagnation is the greatest threat to progress. •Embrace discomfort—learning new techniques, whether robotic surgery, AI-driven diagnostics, or precision oncology, is challenging but necessary. •Be open to transformation—what worked yesterday may not be the best today. While Krishna may not explicitly mention the metaphor of a fish in water, the essence of his guidance to Arjuna is rooted in self-reflection, breaking free from habitual patterns, and striving for growth and innovation. Krishna’s teachings offer valuable insights that are especially relevant to professionals striving to remain progressive and impactful, such as surgeons. Here are three key lessons drawn from the Bhagavad Gita: 1. Rethinking Assumptions Krishna urges Arjuna to see beyond the immediate and obvious, encouraging him to adopt a broader perspective about his duty (swadharma) and the transient nature of life. This principle is vital for surgeons, who must continually question traditional approaches, rethink outdated techniques, and embrace progressive treatments. Just as Arjuna redefined his sense of purpose on the battlefield, surgeons can redefine their practices to improve outcomes and minimize complications. 2. Detachment and Awareness Krishna emphasizes karma-phala-tyaga—the importance of detachment from the fruits of one’s actions. Much like a fish unaware of water, people often become blind to their environment and routines, leading to stagnation. For a surgeon, this means avoiding complacency and instead critically examining current methods, seeking innovation, and embracing change. Detachment allows for a focus on learning and growth, rather than clinging to past successes. 3. Evolving Through Knowledge In the Bhagavad Gita, Krishna highlights jnana-yoga (the path of knowledge) as a way to gain clarity and transcend ignorance. For surgeons, this translates to staying informed about the latest research, exploring new technologies, and integrating innovative treatments into their practice.
