Paperwork does not keep people safe. People do.

Integrity is everything.
Reduce incidents with a 5 step pre-task risk assessment:
1. Why am I doing this?
2. What could go wrong?
3. How likely is it to happen?
4. How could it affect me or others?
5. What should I do about it?
If the risk is too great;
STOP!
Control the risk!
Prevent the incident!
